    I went to this center twice. I was on time, and both times they were an an or more seeing me. When I got called on the first time. They wanted me to sign a bunch or forms for treatment etc, at which time I informed them that I was will to be treated only if my present insurance would cover all expenses with Mo out of pocket. On both visits they did same, I stated same even wrote on my forms such. First visit was more like anatomy course with the provider explaining what was going on with me. I was in pain, assuming that they would give me an injection to relieve my back pain. I was told to come back the following week. Again. I was on time after over an hour, I guess the provider wasn’t there. They decided to first me with a back brace that they. It to fit me. Sent me home. I inquired about the pain shot- they sent me home without any treatment. I decided not to return. The women at the front desk called me. Said that they forgot to do my vital signs and for me to return. I inquired about the treatment for pain. She said maybe tomorrow. I informed her that I wasn’t coming back, she threatened me with over $1000.00 for payment. I can return the back brace still in package that has never been worn. They want the money and keep billing me. I want them to stop billing me and accept the back brace back. I’ve called numerous times. This started in July 2022. The billing company informed me that the provider has been paid. That Medicare would cover Brace. I can return it. I never wore it. Please stop billing me.

    Patient was at the office for an appt and left before seeing the doctor (she had only seen the PT dept) and she took a brace home with her that the doctor has to sign for. Called her immediatly and asked her to come back (within 5 min of her leaving). She refused. Explained that day and documented in her chart that day that insurance would not pay for brace without her seeing the doctor. Advised she needed to come back or return the brace. This occured on 7/11/2022. I told her that if she did not return the brace she would be charged for it as her insurance would not pay. She stated she understood, and would come back in later that week. She never returned to the office. It's been too long now for us to accept it back to the office because she disregarded the instructions and now wants to return the brace. The bill is valid for a product she has and we will not accept it returned now

    I am rejecting this response because:



    The statement is not true.  I was informed 

    after waiting for over an hour that the doctor wouldn’t be in.  That maybe tomorrow I could return and that maybe he would give me something for pain. I was given the Back brace to pacify my not being seen.   I received a phone call after over an hour requesting that I return because they forgot to take my vital signs.     I inquired about seeing the physician for pain. I was informed that he wasn’t available that day. But to return so that they could do vital signs. And maybe I could return on the following day so that he could give me something to relieve the back pain.  I requested that I return when the physician was available.  The clerk informed me that I needed to return so that they could take my vital sIgns and be paid.   I informed her that I could return today.  To contact me when the physician was available.    She then threatened me with the $1000 bill. I informed her that they could have the brace. I hadn’t worn it  that I wouldn’t be returning.     ******* *******

    Ozzie Smith IMAC in Webster Groves, MO
    Transactions dates, 8/19/22, 9/19/22, 10/19/22. There are more charges going back one year but I do not have the credit card statements. I have called on 9/6/22, 9/13/22, 10/3/22, 10/5/22 and 11/28/22 and each time I am told that the account has been paused and that a refund has been processed. I have talked to ******, ******* and ***** who apparently handles billing. As of today, there has been no resolution. I am the niece and POA of my 93 year old aunt whos account this is happening to. They have no explanation as why the charges are continuing and each time I call, they tell me that someone will call back. They never do. I am not sure why they would pause the account and not close it, but that is what I was told. In the meantime, the credit card company charges her a late fee and I have received calls from the Collection Dept. I have talked to Mr. ****** ****** with the bank to explain what is going on. At this time, I have no clue as to what to do except file this with you and get my attorney involved if need be. The phone number for the location that I have dealt with is* ************. I would appreciate any help. Thank you

    Spoke with Ms. ***** *******'s niece who is the POA who states she has been trying to stop the automated payments for the Wellness Program of $55.00 per month. Told her that I would be requesting a refund for July, August, September, October and November 2022 payments but would not be able refund the late payments of $100 accrued through the credit card company and suggest she contact the Credit Card Company directly about that.  The niece was appreciative of the 5 months of refunds since she had originally asked to cancel the program in June of 2022.

    I went to this place for help with my knees. I told them that I had ********. They told me they took ******** patients. I went to them for twice a week for two months. I ended up getting a bill for $1000. They gave me shots in my knees and it didn't make me any better. I tried contacting the office but there is no corporate number they just referred me back to the office. Also while I was there getting ******** *******, the therapist sat on my foot.
    I quit going to them. They claim they submitted to ********, but I ended up with a bill that I can't afford. They told me that ******** would take care of this. I feel like they deceived by them. I don't remember signing anything.

    *** ***** called the office on July 12, 2022 with the same complaint. It was explained to her that on 6/28/2021 she signed a Plan Of Care showing how much her insurance would cover for treatments and what they would not cover. The amount of treatment that would not be covered by ******** was $1,725.00, it was explained in person and written on her Plan Of Care that ******** would not pay for ************ *************. When asked if she would like this form mailed to her she became very angry. Read the notes from ******** ******* with the claim of "siting on her foot" and no where in any notes does that look to be a viable claim. The ********* that treated her is no longer with our company, the other therapists stated that she was the gentlest therapist of them all and would never do such techniques. Our therapists are highly trained and are here to help our patients, not harm their treatments and recovery.  *** ***** insisted on this bill be written off due to it not helping her situation, it was reiterated that no treatment is full proof, that every body is different in how they respond to treatments. She became belligerent and hung up.

    I am rejecting this response because: I would have never started this treatment if I had known it was going to be this much, they did NOT explain anything about that.  I would never have signed off on all of the treatments they had on this plan.  The first thing that came out of my mouth was that I was on ******** and I can't afford any major expenses.  Besides, neither the doctor or PT I had work there anymore.
