      On August 10, 2022, my mother signed a contract for professional funeral services for my father. We paid 4,745.00 for complete services, including applying for the death certificate. Pastor ***** filled out the affidavit for the death certificate at this meeting when he was hired for services. On February 18, 2023 at 11:31 a.m. I called Pastor ***** because we had not received the death certificate. Pastor ***** stated that the social security number was entered incorrectly and that he needed to complete an affidavit to have the social security number changed on the form. He was given the correct social security number over the phone at that time. By the beginning of March, the family had not heard back from Pastor ***** or received the corrected death certificate from vital records. I called Pastor ***** again on March 6 and at this time, he was emailed a copy of my father's social security card. I spoke with Pastor Clark and he stated that there was a backlog of applications for certificates at vital records, but he was sure it wouldn't be much longer. Unbeknownst to Pastor *****, my sister, ******* ******, had spoken with "*****" at vital records, who confirmed that there was a backlog of applications, but the team was working on April applications. "*****" searched the database for an affidavit to have the social security number changed for ******* ** ****** and had not received one. He confirmed that there is a death certificate but no change affidavit. The social security number is, in fact, still incorrect. I spoke with Pastor ***** on May 1 and May 15 to resolve his mistake and to get the corrected death certificate. He continues to blame vital records stating, "I have documentation that an application was submitted to vital records for the social security number change." We have not received the corrected death certificate.
