      I received a bill from AAA Ambulance Service for a trip to a procedure on 08/10/2023. The bill was received many months later. I was told a *** ( certificate of medical necessity) form was needed from the provider in order for them to bill the insurance company ( ************). The provider faxed the form to *** on 07/24/2024 which was verified received by personnel in the business office.I am seeking help to resolve this issue because *** turned this bill over to ********************************* A Debt Collector. I spoke to **** at *** business office and its apparent to ME that they never submitted the *** form to the insurance company for payment. She told me that they would submit the form for payment to the insurance company but the bill would remain in collections. I spoke to ******* with collections and was told that *** could request a hold on collections until the insurance is billed.I am asking BBB to have upper management address this issue in order to ensure my insurance is billed for this service and for AAA to place a hold on collections until the insurance process the claim.Thank you in advance for assisting me with this matter.The debt account number is *********** reference number for the debt collection is: ******** Sincerely,*** ******

      A Certificate of Medical Necessity (CMN) was needed for this trip. The provider did submit documentation to support however it was not on the *** form and completed in its entirety. We did not follow up with the provider's office to let them know what exactly items the insurance company needed. We filed the claim with insurance with the provider's documentation. It did deny for timely filing, we sent the claim to timely filing. The customer is not responsible for this claim. It was sent to *** in error. The customer has been removed from the *** Collection agency.
