    On August 21, 2024, my daughter, attended a therapy intake session with licensed clinical social worker ******** ****, who is part of Moxie DBT, PLLC. Since Moxie DBT is out-of-network with my insurance ******************* Blue Shield of **********), I paid the $120 session fee directly to Moxie DBT, PLLC. I was informed that I would receive a superbill containing all the required information to file a claim for reimbursement.However, the superbill provided by Moxie DBT was missing the business's Tax ID number. Without this number, my insurance cannot process the claim. I have repeatedly contacted Moxie DBTs administrative assistant at ***************************** phone: ************), and provided documentation from my insurance showing that the Tax ID is required. Despite this, the information has not been **************, a Horizon representative contacted Moxie DBT, PLLC to request the Tax ID, but it was again refused, with the explanation that Moxie DBT does not share their Tax ID and other clients are reimbursed without it. This refusal is unusual as other therapy groups, such as ******************************* and ******************************************************* routinely include their Tax ID numbers in their bills, demonstrating that sharing this information is standard practice.I urgently need Moxie DBT's Tax ID to receive reimbursement for this claim.Attachments:1) Horizon medical claim form highlighting the requirement for the Tax ID (bottom of the form).Thank you for your attention to this matter.

    It is the policy of Moxie DBT, PLLC, to provide a "superbill" for all out of network services, which clients can choose to submit to their insurance companies for reimbursement. The superbill is a detailed receipt including date of service, CPT codes, diagnosis codes, as well as the provider's NPI number. It has been our experience that when a tax ID is provided, the reimbursement may be paid to the provider instead of the client, which creates hassle and confusion. However, we work diligently with our clients to ensure they have all the necessary information to have their claims processed and we will continue to do so. We value ethical practices and client satisfaction while we understand that every insurance carrier may have different requirements.
