    On May 12, 2023 I was the beneficiary of the deceased of **************************. This company allowed the daughter ********************************* to take control since their was a relationship. Burial arrangements and everything was done by ******. The Informant on the death certificate was myself. ****** is the heir not the Informant.

    The daughter provided the information and contracted with the funeral home. Her name is listed as the informant for that reason. An amendment can be done with ************************************ department.

    I am rejecting this response because:

    I was the beneficiary of my 40 years old partner who has passed away on May 7, 2023 and buried on May 12, 2023. *************** ******* daughter ********************************* told him about this funeral home after she used them to buried her husband ***************************** in 2022. After the funeral, *************** ******* and myself made arrangements to be buried here together in 2016. On April 25, 223 the daughter draw up a power of attorney against ***************** wishes and use that information and his stolen I D and SS card after his death. This entity allow the daughter to take over the arrangements and make all the decisions without my consent and allow her to run up the bill to over ***** on a ****** policy . ********************************* allow the daughter to sign the death certificate without a signature of a doctor. We called her about that, she responded by saying you can ammend the death certificate. ***************** didn't die of a natural causes. He was given a morphine shot from the daughter and he passed immediately. The daughter then took me to her apartment for a week in order to arrange the funeral on her behalf. This business use me just to get paid. I would like this business to refund half the price of the funeral I never had a say in. The death certificate has been ammend with correct information. I will be seeking legal help with this Unfair Trade Practices under North ****************** Laws.
