      CHS incorrectly coded and billed me on Aug 2, 2024 for a routine procedure done on May 21 which the insurance pays in full. I first contacted my insurance and they told me CHS coded the procedure incorrectly and call them to have it corrected. I also called Carolina ********* which performed the routine procedure and also told me to dispute with ***. When I first called ***, they said they would have a supervisor call me back which they did not. I received another statement on on Sept 1, so I wrote a hand written letter and mailed it in. These instructions were on the back of the billing statement. Says to write a letter and you do not have to pay the disputed amount while they investigate and can mot report me as delinquent or take any action to collect the disputed amount. I then received another statement Oct 16 and contacted them. The lady on the phone says I see in your account notes they have the letter and is investigating it. Well I just received a bill from a debt collector. They never contacted me regarding me disputing the bill or sent me any correspondence with the resolution. I better not receive bad credit because I have great credit and always pay my bills.

      A surgery with anesthesia was billed to our insurance June 16, 2023. We never recieved a bill for the remaining cost (>$1,500), and instead were sent directly to collections. When I contacted our insurance company, they let me know that the insurance had been billed incorrectly and the provider needed to resubmit. After a LOT of running around (because the bill was already in collections), the provider did resubmit, and our insurance covered the bulk of the bill. We were told directly by CHS Anesthesia that a new bill would be mailed to us. This occured around July 1, 2024. Instead, the bill remained in collections. I cannot get any explanation from CHS Anesthesia why we were never mailed billed and why they sent us to collections directly after insurance disbursed payment to them. In the *** attached, you can see where the provider is expected to bill us for the remainder.

      My husband was never even in ********* on the day they claim we had services done. They charged us $920 and even sent information to insurance. We left the area more than 3 years ago and we got the bill in the mail at our new address. We need the amount to be taken off fully and to stop billing us erroneously. It is not the first time that the medical system has put us into their system erroneously. This is the first time we are being charged for a service that never happened.

      Anesthesia service provided on 4/24/2024. CHS sent two separate bills with the same code to the insurance company, resulting in two co-pays for the same service. Multiple calls to CHS over 30+ days, with no resolution. They keep asking for more time, while the account is now past due. According to insurance company, CHS has to resubmit the claim as a single bill. They have failed to do that. Atrium Health has refused to assist, even though CHS is a wholly owned subsidiary of Atrium Health, citing that CHS bills separately. The two companies continue doing business together.

      12-20-21: surgical procedure Prior to procedure, health deductible had been met so there was zero out of pocket as of 12-20-21 Preapproval was required prior to procedure and was approved. 10-23-23: FIRST BILLING from CHS Anesthesia received for $1,************: I sent a letter (as I was not able to reach anyone via phone after several attempts). Their billing statement indicated disputes must be received within 30 days of billing. 1-12-24: Received another statement for $1,960 1-16-24: I resent my letter 2-22-24: Received another statement 3-11-24: I resent my letter as priority mail.The dispute: It was never disclosed to us that an out-of-network anesthesiologist was used. We would have requested an in-network anesthesiologist or cancelled procedure all together. The fact that our deductible had been met, we had a zero copay, and the entire procedure was pre-approved.Furthermore, CHS Anesthesia did NOT reach out to our insurance (BCBS OF SC) until November 9, 2022 - 11 months after procedure. At that time, the claim was denied because they were out of network. They resubmitted the claim again to BCBS OF SC on August 26, 2023 (almost 2 years after procedure). This time they submitted as in-network. I feel I should not be responsible for this bill. As stated above, all was pre-approved prior to procedure.My letter to them was sent 3 times. At no point in time have they tried to reach out to me.

      Anesthesia provider for outpatient surgery on 12/12/2022 sent two separate bills (one for the anesthesiologist -and one for the nurse) to my insurance carrier with the same code. I asked them to re-submit as one bill because the insurance carrier would not accept two anesthesia bills with the same code on the same date. They refused and said it was my problem to deal with the insurance company. I'll also be filing a complaint about the insurance company because in no way should the patient be responsible for $987 due to the inability of the provider and insurance company to work together!
