Daily visits. Examples of concerns, particularly regarding the quality care and management, are as follows.1) If the Care Plan Team meeting took place, my sister and I were not privy to it or a copy of the care plan. 2) Staff issues- a) working double or split 8-hour shifts; b) pulled from first when more coverage was needed on other floors; c)no one at the nurse station at all times; and d) response to the call button usually not done in a timely manner. 3) My sister continued to complain about being so cold and she wanted the door kept closed due to the draft from hallway. Despite inquiries, this concern was not addressed. 4) I inquired about why my sister was left in the dining room area and was so cold for approximately 2 hours. The nurse commented so many inconsistencies there.and (they) are afraid she may slide out of her chair and wanted someone to keep an eye on her. Furthermore, because she always kept her door close, staff members cant check on her. 5) If the hip X-rays requested by orthopedic fracture clinic were done neither I nor the clinic received the results.6) Because my sister had not shown up for her 8:00 appointment and no one answered the phone, I drove to the facility. She was still asleep; and the driver stated that her appointment wasnt until 10:00. I expressed my concern to the Administrator but to no avail. 7) My sister was being discharged from therapy; yet, the assistant continued doing the assessment and my told her she was exhausted. I know, she keeps taking breaks. And if I wanted her to stop, fine then, you can take her back to her room. 8) I also found the actions of the social worker involving my sister giving her her personal items (e.g. purse, wallet, checks..) extremely troubling.9) Per the Administrator, even with Power of Attorney documents, my sister can make her own decisions; therefore, they were not obligated to keep me informed. And they did not!Respectfully submitted,***** D. *******
