- SPA FITNESS CANCELLATION POLICY
CANCELLATION PROCEDURES FOR
MEMBERSHIP/ CONTRACT: You must meet one of the following cancellation
requirements to request membership cancellation: 1) Disability; 2)
Death; 3) PCS Orders or Permanent Move to a new area where you are
thirty (30) miles or more from any SPA/ GYM willing to honor your
membership. ETS Orders follow the same procedures as a permanent move.
TDY Orders may qualify you to make up time missed on your membership due
to being out of the area, but do not meet cancellation requirements.
All requests for cancellation must be in written form. Written requests
are to include a summary of the facts supporting the cancellation
request. Written requests will be reviewed by a designated panel to
objectively review facts. If a legal opinion is necessary, legal council
will review request and recommend action to be taken. Review panel
meets every Thursday- written requests are normally answered with in
thirty (30) days. Upon request you will be mailed a cancellation form to
be completed and return to this office. You must continue to make you
monthly payments in a timely manner until cancellation request is
approved. Your MEMBERSHIP/ CONTRACT remains in Full Force and Effect
until you are notified in writing that your Membership/ Contract has
been canceled. If MEMBER cancels bank/ credit card draft prior to
MEMBER'S account being paid in full, MEMBER will pay to SPA, or holder
of promissory note, the $5.00 per month discount that MEMBER originally
received in consideration of bank/ credit card draft. Said $5.00 per
month shall be due and payable each month on the payment date of each
remaining monthly installment. That is, if MEMBER cancels bank/credit
card draft, member's remaining monthly payments will increase by $ 5.00
