************ was admitted to Maggie Valley Nursing & Rehabilitation Center on September 2, 2022, and went home on December 10, 2022 for a total of 99 days.  He was admitted with 100 days of ******** coverage which is 20 days paid at 100% and the remaining days paid with the exception of co-insurance which at the time was $194.50 per day. When admitted, ************** had 99 days available,  he had 20 days covered at 100% by ******** and 1 Day covered by his insurance (see explanation of benefits page 1)

He did have ********** Blue Shield of ** which in his case pays from day 21 to day 30, and his insurance was billed, however, all that ********** paid the facility was one day. The Explanation of benefits stated that the maximum number of services have been allowed.  We would not know where he used the other 9 days coverage prior to being admitted here. This leaves the account responsible for the remaining days of co-insurance.

I'm really not understanding why ************* is reporting this to you, the account has been zero since June 2023 at which time, all of the co-insurance due on this account was written off as a bad debt and she was sent a statement showing a zero balance.  I have attached the statement for your review as well as a transaction history showing the write off on the account.  If you need any additional information, please do not hesitate to reach out to us.  ***************************, Administrator
