    My name is *************************, and I am filing a complaint against a medical clinic for fraudulent billing practices. On 6-21-22, I had a scheduled yearly checkup that was covered by my insurance. During the visit, I discussed several symptoms and issues with the doctor, and we went over my medical history.However, a few weeks after the visit, I received multiple notices that I owed $315 for the services provided during the checkup. I was surprised to receive these bills because my insurance was supposed to cover the cost of the yearly checkup. Additionally, I was asked to sign a financial agreement form to agree to pay for anything not covered by my insurance, which I never signed.After further investigation, I discovered that the clinic had changed my appointment type in their system and billed me for three separate visits instead of the yearly checkup. This was a deliberate attempt to overcharge me and commit insurance fraud. I believe this is a clear case of malpractice and theft, as I did not receive three separate visits and should not have been charged for them.I have attempted to resolve this matter with the clinic directly, but they have been unresponsive and have not taken responsibility for their actions. I am filing this complaint to hold them accountable for their fraudulent billing practices and to prevent them from doing this to other patients in the future.I have attached a PDF documenting the picture and files and email related to this as evidence of such.
