    I am facing issues with my disability paperwork. It has been delayed due to ************ **** not researching and accusing me of lying and only going to physical therapy once In 2024. I provided proof of my attendance through bills and screen prints sent through the Novant health portal. The office has not apologized for the mistake. **** advised the physical therapy office, Athletico Pivot, to not make any more appointments for me which stopped and held up my healing. I showed a stack of papers to show ************ regarding my daughters mental healty crisis which corresponds with some of days I had to cancel. Also, Pivot always allowed me to make up my PT therapy appt the same week. No pro lems there. However, ************ refused to look at the paperwork and showed an unbelievable lack of compassion. The records in the portal are inaccurate and sometimes do not include notes from calls and show incorrect no call no show. I filed a grievance on May 6th. I received a rude call from a staff member on May 8th stating don't call them anymore and dont add any paperwork to the portal. My concern that needs to be rectified is about the delay in HR receiving the correct paperwork and the impact it has on my ability to access physical therapy and receive pay. ************ **** needs to send in corrected disability paperwork per my hr person . ************ needs to fill out all necesssry paperwork from February till the end of april with original date of diability 10/15/23,when i had major surgery on my right ankle. She also has to release me to go back to work on letterhead to HR *************************** ******************* This is bc she filled out disability paperwork May so the January letter releasing back to work is no longer valid. She needs to do this as I was under her care during this time not the doctor she is trying to refer me to.

    I have not heard from the business in response to my complaint.  The doctor sent you a picture from the portal, from Athletico saying I haven't had any PT Appointments there. She is correct. You can reach out to them , b/c the last time I talked to them to make an appointment they were advised by the doctor to not make any future appointment for me. I no longer have trust in that particular Physical Therapy place as ************ has tried to make it apparent to them that I am Not working on my physical  therapy which is not true. Also she would not even look or ask anything about my foot, even when I called In. If you listen to the phone calls made to them, you will understand.  She has not made any attempt to fix this.
