      Dr. ********************** **** practice did evaluate-problem focused and charge me 01/25/2024. On 01/29/2024 the office contacted my insurance company to file a claim for cleaning. On 02/02/2024 I came in for the cleaning I was charged for a periodontal scaling/root plan on 1-3 teeth and 27 tooth also charge for periodontal scaling/root plan for 1-3 teeth and 21 tooth plus for TOP FLUOR VARNISH; APPL MOD/HIGH RISK. I that time I was led to believe the insurance card I provided was to cover the cost. Shortly after the procedure was completed, I get a bill, the insurance company did not paid for those procedures. Also, my insurance company was out of network. My dispute is the practice fail to inform me of any of out of network and the Quartly time frame. of the insurance time to receive payment. Also, I told the practice I came from another dentist off to compare prices for upper dentures. The practice did not check with the other dentist office for any work that was already done before they did the cleaning. The result was I was left paying a bill that could have been avoid if they would have let be me know before completing the procedure.

      In our patient paperwork that the patient signed, states that all proposed treatment is just an estimated and not a guaranteed of payment from your insurance company. If you want to know the exact amount you would be responsible for, you would need to wait to schedule until the pre-estimated is back from the insurance company. The patient's insurance did deny both visits but since then has came back and paid for the patient's first visit in full and is still pending for the second visit. Not sure why he is saying he is responsible for all procedures out of pocket since the patient should be able to look at his claims either online or via mail to see that they came back and paid for the first visit. As far as his records, we did request his records and the only thing they sent over was xrays and a proposed treatment. We are not out of network with his insurance not sure why patient is stating that either. If you have any questions or concerns, please feel free to reach out to me.
