    On March 2, 2023, I was evaluated for a DWI assessment. I paid a total of $740 for both the assessment and DWI class. On July 6, 2023 I finished the zoom class and received a certificate. However, Knewera Consulting was required to submit Form 508 to the department of health and human services as proof that I was assessed and received the recommended treatment. I have been asking since July 2023 for them to provide this proof. They have neglected my requests. I have also sent reminder emails and made several phone calls. On June 25th, 2024, I received a letter from the ************* of transportation saying my driving privilege is suspended due to Knewera Consulting's failure to send this document. I have since called many more times and send emails almost every day. They would state that they will send it but they still have not. I need this document send to renew my driving privileges. ****************** responded once through email but have since ignored my requests and did not follow up nor answer the next emails.
