    I signed up for Hormone Replace Therapy Agreement with Restorative Health Omaha on 02/02/2023. I received 1 testosterone pellet injection on Feb. 7th after going over my labs. I had a follow up lab on 3/20/2023 and was given a 1/2 pellet.

    On May 1st I contacted **** **** , Medical Consultant on the phone and told her I wanted to cancel my services as the hormones are impacting my health negatively and that I want a refund. She told me that I am not entitled to a refund and that she "wished I would continue to come since I have paid for it."

    I told her that was not true that I know the paperwork I signed that broke down the cost stated that after 1 treatment I'm entitled to a refund in XX amount. She said that's not true. I told her that I know it's true because I know what I signed and that I would contact my attorney. She said okay.

    About 5 minutes later I received a call from her on May 1st at 10:00am I sent it to voicemail, she left a message. "Hi hello ******* it's **** from Restorative Health. My eyes are not fast enough but you are correct this would've only been your second implant not your fourth so your entitled to a refund.

    I called her back she told me she should process the paperwork and I would be refunded in 30 days on June 1st. I protested, but she said 30 days is their policy.

    Fri. May 5th I went up to the office to request a copy of the refund paperwork she submitted to "Corporate." She was not in and the phlebotomist gave me a copy of my agreement which **** didn't provide me. She however, couldn't access my payment paperwork I signed which outlined the refund policy and how much is given back after different treatments, but she would have **** get it to me on Monday.

    **** hasn't given me a copy of the paperwork. I have emailed her several times today June 1st being the most recent as my credit cards have not been refunded the nearly $1800 I am owed.

    I am demanding the refund I'm due be processed immediately by them.
