      on 10/24/24 Ihad a eye exm. with this office. I had called and asked for a price for a routine eye exam. and was quoted $190.00.after the eye exam. they filed a claim of $284.00 with my health care provider that was denied on 12/10/24.then I receved a bill by mail dated 12/27/24 for $284.00 plus a $41.00 late fee.I have called twice 01/03/25 and on 01/06/25 to resolve this matter but have not had my calls returned.This bill is dated 12/27/24 that is 11 days ago and there claiming a $41.00 61-90 day late fee as well as a nearly $100.00 more for the eye exam. then quoted.

      This is to inform you that the billing dispute with Millard Family Eye care has been resolved.  I received a call from there office manager who handled this dispute in a very professional manner.  That exceeded my expectations in this dispute.  I would like to thank you for your intervention to help me with this and also Thank Millard Family Eye care for there more than fair response.
