I signed paperwork for a "H3338353034323636****H Payment Plan Agreement", "H3338353034323636****H Treatment Consent Form", "Photography Release Form", and General H3338353034323636****H Treatment Consent Form". During the next few days I brought in cash in the amount of $280 down payment. I was excited about the possibility of solving some of my H3135323431**37303032H issues through this plan.The paperwork does NOT indicate a "repayment plan in part IV" except to say under d. payment instructions "Until paid in full, partial will not be delivered to the patient + balance ($2288 total)" 1. There is no notation of the deposit of $280 I made in the days following the signing of the document. Just prior to my visit to your office, my life partner passed away suddenly. This shocking event is still affecting my life in a number of ways. This change has required me to significantly realign my goals and future plans. I was required to rapidly move as I could no longer afford the apartment I had lived in for over 10 years. My finances were severely affected as all bills now became mine alone. The Thanksgiving Holiday passed in a blur. In December, realizing the extreme financial difficulties I was facing, I called your office. The person I spoke to (not sure if it was the receptionist or ******) stated that I could no longer afford the procedure that we had planned. I was told that "it shouldn&#**;t be a problem to cancel the procedure and get my money back". Since then, I have received numerous calls from a collection agency regarding the debt owed to you for this procedure. I have had no procedure done and there was no set schedule of payments established in the "H3338353034323636****H Payment Plan Agreement". I have called a number of times regarding this difficulty. About a week ago I called and was told " that my deposit had been refunded to my account". I got thinking about that after I hung up, How could my money be refunded to my account when I paid in cash?". I visited your offices on March 6, 2025 and spoke with ******, your office manager. She stated that she understood my concerns but that " the owner was the person who told her to send my bills to the collection agency and that only the owner could make any changes". After examining the paperwork more closely, and searching resources about Fair Credit and Billing Act, I am writing this letter to you. I state the following: 1. I cancel the plan to work with you. ( based on section #7 of the "General H3338353034323636****H Treatment Consent Form" I am expressing my right to choose from alternative treatment plans - in this case - I am choosing NOT to have the procedure done.)2. I dispute this debt. 3. I dispute that I owe you the full amount of the debt. 4. I dispute that you should have placed this amount in collection and affected my credit score. Under the Fair Credit Billing Act, I have the right to dispute and withhold payment for goods and services I have not yet received. I hope that you understand my problems and concerns and will allow me to cancel this procedure and stop the collection procedures you have started. Thank you for your attention in this matter.     1, Date of the Preventive tooth cleanup: 08/06/2024 2, The amount of money paid by my insurance: $76 3, What Gate City Dental Associates committed to provide preventive tooth cleanup fully covered by insurance card twice a year. My 16-year-old son was transferred from another dentist after double confirming the service fee was fully covered by the insurance company.4, but I got a bill for my son and need to pay a balance of $94 except the $76 paid by the insurance company.5, There was a topical fluoride varnish service fee ($50) from the bill my son never did before. The nurse suggested my son do the service without notifying him there was an additional charge.6 I have done the preventive dental services 3 or 4 times and the business never charged me an additional fee since the insurance company 100% for preventative care services. Why are they charging my son extra? By the way, I want to complain about their service. Around July, I had an appointment, and their system automatically sent me several reminders to confirm the service. I confirmed it, but when I rushed over, the office was closed. I made countless calls, and the automatic messages on the phone said that they were serving other customers. There was no message on the phone saying that their office was closed, nor did they say when they would open. I really felt that they were very unprofessional. This time the fee was also inexplicable. I confirmed that the insurance could cover two teeth cleanings a year before I transferred my son over. When I called just now, they refused to cancel the bill, saying that the service had already been done. I had no choice but to cancel the subsequent services, and I also hope that they will cancel the $94 fee.
