      On January 24, 2024 I had an appointment at Seacoast Dermatology for a small rash on my finger. As a secondary item I mentioned to the provider, ***********************************, that I had a small skin tag on my right side rear thigh area and was wondering if it could be removed. He asked me a number of questions around how bothersome it was, and if it gets caught on anything, to which the answers were all "no". It was purely for cosmetic reasons I told him. He said it was not something that could be submitted to insurance, so it would cost $100 out-of-pocket. I said that's fine, please do the procedure. When I checked out, I offered to pay the $100, and the receptionist said I would receive a bill in the mail for the $100.When I received a bill for the visit I was quite surprised that the skin tag removal had been submitted to *************************** for $369.36, of which I owed $188.14. The notes in the bill and on my medical record were completely false. First of all, the location of the skintag as noted in the chart was falsified and moved to the front waist of my body, a completely different area then where it was, and the notes said that it was medically necessary to remove it. That was a complete lie.Upon doing some internet searches (many, many reviews of the exact same billing behavior on Yelp and ******* it appears that this is a very common practice of Seacoast Dermatology, to tell patients that there is a minimal out of pocket cost for a procedure, and then submit it to insurance for a much larger amount. This is wrong and fraudulent insurance behavior. I have tried for 5 months to contact both Seacoast Dermatology in **********, ** as well as their billing office (Forefront Dermatology) in *********, ********* and they will not change the billing. This company is fraudulently billing insurance companies for procedures and needs to be investigated.Thank you

      I had filler (Sculpltra) placed in my cheeks on June 24, 2021 and was charged 425.00 however the later bill added late fees. I called the office stating I was sending in a check for the original 425.00 that was agreed upon. The billing agent stated that she was still re-sending out the new bill with the late fee. I sent out my check for the 425.00. Today I received a new bill with the late fees from the filler however they made it look like it was for lesion that was never on a previous bill and is the same amount $82.12 that they were charging for the late fees. They are dishonest and I'm shocked to see that they are now attempting to make the bill look like it was the customer's portion after insurance paid their portion. The billing office stated recently that they were charging late fees. I said that I would dispute them and billing office re-sends out the bill with the same additional late fee disguised as what the insurance company didn't pay for another visit.

      On 11/29/2022 I had an appointment at Seacoast Dermatology in Portsmouth, NH for a minor, not urgent condition.

      Upon checking in for the appointment I asked the person working at the front desk what my total out of pocket expense would be for the visit after insurance was billed. I acknowledged that they might not know off the top of their head and that it might require contacting my insurance company, I was happy to wait or reschedule if needed as I was not there for anything urgent. The person working at the desk pulled up my insurance information on file and said that the $35.00 copay would be my total expense. I asked again to make sure that this was the TOTAL amount after insurance and they confirmed again, $35.00. I paid my copay and proceeded with the appointment.

      A few weeks later on 12/12 I received a bill from Seacoast Dermatology for $363.39. I was shocked and disappointed as I had been very explicit in my question at the time of my appointment in order to make an informed financial decision about whether to proceed with the appointment. I understand that oversights occur, I think (and truly hope) that the person working the front desk that day was not trying to be deceitful but made an honest mistake. That being said I do not think an oversight on the part of Seacoast Dermatology by giving me incorrect and/or incomplete information should cost me hundreds of dollars.

      Upon receiving this bill I called Seacoast Dermatology's offices and spoke with the Office Manager. I explained to the manager what I laid out in the paragraph above. The manager was defensive, insisting that $35.00 was an acceptable answer and no mistake was made because it was what I owed "at that time". Again I explained that my question was not what I would owe that day, but I had explicitly asked what my total out of pocket would be. If person working did not know/could not find out, an honest answer of "I don't know" should have been given. Manager refused to take action.

      Forefront Dermatology takes all
      concerns seriously and we do appreciate you bringing this to our
      attention.  Unfortunately, at the time a visit is scheduled we are unable
      to provide an exact quote of what a patient’s financial responsibility will be
      without knowing exactly what services the patient will need at the time of the
      visit nor do we have visibility into any deductible or out of pocket amounts
      that may have already been satisfied under a patient’s benefit plan.  We
      have reviewed the relevant information associated with this concern including
      our Patient Communication and Financial Policies form signed by Ms. ******
      which states the following: 

      "Insurance Filing: As a courtesy we
      will bill your insurance company for charges incurred at our clinic. Please
      remember your health insurance is a contract between you and your insurance
      company. Our office will make two attempts to settle any outstanding bill with
      your insurance company. If your insurance deems a service to be not covered by
      your insurance plan you agree to be responsible for the balance of this
      service. Claims not paid by your insurance carrier within 90 days will be
      considered a non-covered service. We will furnish information required by the
      insurance company to receive payment. Benefits should be paid directly to the
      Practice from your insurance company. If your insurance company reimburses you
      directly for any outstanding amounts due to us, payment will be expected by us
      within 10 days. hereby assign to Forefront all my rights and claims for
      reimbursement under my health insurance policy. I agree to provide information
      as needed to establish my eligibility for such benefits.

      Procedure Pricing: I understand that
      procedure estimates are only provided in writing.  Written estimates must
      be requested prior to the appointment."

      Additionally,
      a review of the details included on the Remittance Advice from Aetna U.S.
      Healthcare confirmed that the bill from Seacoast Dermatology for $363.39 was
      applied to Ms. ******’s deductible. As noted above, given this is a component
      of her plan benefits contract with Aetna, we are unable to waive these
      charges.   

      We sincerely apologize for any frustration or confusion
      with this process.

      I do not accept the business's response as a resolution to my complaint because: The business is simply restating what they have been saying all along and not actually responding to the matter at hand. Maybe their policy is that they are unable to provide quotes or accurate information to their patients (I have never heard of this at any other medical office). Regardless, the issue I would like rectified is that I clearly asked at the time of my visit what my total out of pocket would be after insurance. If they lack the ability to accurately answer the question they should have said so at the time, not tell me it would be $35 dollars and then follow up with a bill for 10x that amount.
