    On or about August 30, 2024 I hired **** Moran (Independence Therapy Associates) to perform a functional capacity assessment. **** assured me of his expertise in both performing the assessment and in writing a report that would document and support my disability. To the present date I have asked **** for the report on more than two dozen occasions. He has promised it to me more than a dozen times. Every time he has an excuse for why he hasn't written it yet. I've asked him if he needs more information, if there is anything that I need to provide him to finish it. I've told him of the urgency and that my stability is on the line. I've made it clear that the delay is causing me enormous anxiety. I don't want a refund, I want the report that I was promised. This is threatening my security and causing me extraordinary distress.

    ********, Attached is my reply with signatur.  We would appreciate another reach out if possible by phone, and email.  The non response to your efforts is the issue we've had.  If they continue to refuse to reply I want to make sure the complaint remains on your website and is posted against the bureau ratings. It is important to us that professional business firms, and medical providers be accountable.  We have made more than enough efforts at resolution and contact, in fact much mor than ever should have been necessary.  

     

    FYI, that this NH firm also operate a second clinix known as "The Dracut Hand Therapy and Occupational Wellness". It is also run by **** Moran, who conducted my occupational functional capacity testing.....  That offices info is listed below in case this helps tracking them down.
