    family was tested at "**************************" during the Covid-19 crisis in late 2021. TODAY (11/21/24) I received threatening "past due" emails stating I owe $150 per test for 6 tests. There is no phone number to call and the "send email" button on the ********************************* states "ALL SITES ARE NOW CLOSED Thank you for allowing us to serve you the past 2 years! If you have a question about a test, please reply to one of the emails we sent you for your appointment or test results." The email in question contains my name and birthday in plain, unencrypted text in an attachment; fairly certain that alone is PHI and a HIPAA violation. This feels like a scam- someone coming after me with threats of ruining my credit after 3 years with no one way to speak to someone?

    In Dec. of 2021, My husband returned from a 1 year deployemnt overseas with the ***** We were on our way to a celebration cruise but needed covid tests. We were charged ****** per covid test and we needed 5 of them. 2 of us came out positive and we had to cancel our celebration cruise. However, this is where the fun starts. First call does not take insurance but gave us the paperwork necessary to submit to insurance, which I did. My claim was denied because first cal llc did not have a proper "certification application". ***************** they did. There were numerous emails exchanged and phone calls. My insurance provider says there were PAGES of claims they had not paid because of this same issue at this same lab. I filed 2 appeals, 1 kepro investigation with my insurance and a complaint with the BBB for Tricare Humana as well. However, ******* was able to successfulyl certify the locaiton 2 years later. So they were indeed working without a certification application. The business is since closed down and we can not contact them in anyway. I know there are sister companies though. Tricare did indeed send me a pathetic payment for ***** per claim rather tan ****** ... Tricare or first cal LLC owes me the rest. They are severly overcharging and they were operating without a proper license. 245 times 5 is ***** for reimbursement. Evidence avilable upon request.
