I am writing to formally dispute unauthorized charges related to an initial consultation my partner, **** *****, and I scheduled with your Florida branch in [Month/Year].

Last October, we contacted your office to request a consultation, during which your representative explicitly stated the service would be complimentary with no mention of any associated costs. Relying on this assurance, we proceeded with a 20-minute Zoom call on [date]. Notably, the consultation was not conducted by a Florida office representative but by an individual acting on their behalf.

Despite being assured of a free consultation, we received two separate invoices:
      (1) A bill for $164 was addressed to me, which I paid on December 30th, 2024 under the assumption it was an error, hoping to resolve the matter amicably.
      (2) A subsequent bill for $186 addressed to my partner, **** *****, dated [date], for the same consultation.

This is unacceptable for the following reasons:

Misrepresentation of services: The consultation was advertised as "complimentary," with no prior disclosure of fees during the call, in writing, or via consent forms.

Duplicate billing: Charging both parties individually for a single joint consultation is unreasonable and deceptive.

Lack of transparency: No explanation was provided for the differing amounts, nor were we informed of any billing policies prior to the call.

Thus, we demand:(1) Immediate reversal of the $186 charge for **** *****. (2) Refund of the $164 I paid, as the service was misrepresented as free.

Written confirmation that our accounts are cleared and no further charges will be issued.

Attached are copies of the billing statements for your reference.

I was wrongly sent a bill by this business a few months ago. In spite of categorically informing them multiple times that I was being sent the bills in error, the "alleged debt" was sent to a debt collection agency, ************, **** Upon receipt of the "debt collection letter" from ************, ****, I contacted this business MULTIPLE TIMES and was EVENTUALLY informed by their supervisor that the bills I was being sent was indeed due to an error on their part, and since then it has been rectified. I was also told by this supervisor "to call/contact ************, **** and clarify this.I am not satisfied with this resolution because IT SHOULD NOT BE MY RESPONSIBILITY to contact a debt collection agency about an "alleged debt" that originated due to errors in billing committed by this business! I do not know what "rectification" in billing means but I want that this business contact ************, **** and clarifies with them that this "alleged debt" should never have existed to begin with and that this "alleged debt" has been rectified to show a ZERO BALANCE due by me/family. I SHOULD NOT HAVE TO SPEND MY TIME/RESOURCES AND SUFFER CREDIT CONSEQUENCES BECAUSE OF A BILLING MISTAKE ON THEIR PART.

RMA failed to have all of their systems update my address change. They made zero attempt to call or email about an incorrect address before sending to collections, who somehow easily had my address.

On 1/16/2023 I went to Dr. ************ office (RMA in Abington, PA) for my 4th US, at his direction. The other three US were all positive and after every visit there was a "problem" created by his incompetent/unattentive nursing staff. I was called later in the day by his RN and told they won't help me with the next step until I pay my bill of $200 and stated even if I paid that day I'd need to have another US and blood work next month. I yelled at them and stated I was leaving the practice and wanted my records. They said no and sent me an email that stated I needed the pay for my records via credit card. My new practice tried to get the records and RMA refused the requests. I cannot get pregnant without these records and I need the money my insurance company and I wasted on this company back so I can start over with UPenn. I'm not giving them my card and no other practice has ever tried to charge me for records, or used my diagnosis as a means to extort money from me. My account wasn't even delinquent, I just made a payment in December 2022.
