    Received a bill for $40 from Palisade Eye Associates on 03/22/2025 for my visit to their office on 03/27/2023 (2 years ago!) for the co-payment for a test that was not fully paid by the insurance, however I have never received any explanation of benefits from the insurance company. I paid the bill to avoid its submission to a collection agency (that is what the company said they would do, if I don't pay the bill), but I feel that the bill was issued in error.

    2) Envelope from the bill postmarked 03/20/205 for the bill that was due on 03/06/2025
    3) My receipt for the payment of $40 to Palisade Eye Associates

    I have attached the explanation of benefits from the insurance company advising the approved amount and the patient's co-pay amount. **** ***** **** ****** paid in the amount of $2.83 leaving the copayment to patient. The first statement was sent out to the patient on April 10, 2023.

    I have never received an original bill from Palisades Eye, which they claim was sent on April 10, 2023.

    Please have the company provide the original invoice dated 4/10/2023.
