    On 4/11/2023 I went to the Pediatrician ********** in Hillsbrough **. I went to request my son's medical records from birth to present. I didn't give any authorization to anyone or any other organization access to my son's records. I walked back into the office, They then informed me that my sons record, and *** had left the office and I couldn't get anymore information. and the docs were being transported to the Headquarters. I then asked where was the headquarters located? They said the ******* off I then went to that location.I asked did they receive my son's authorization medical records forms? They said that my records will be ready 4/12/2023 after 10 am. On April 12, **************************************************** the doctors office. The envelope had a label on it that spelled out medical records pick up ******* office parents(s) ************************* 9-14-2011? which is questionable why not my name? 1.There was an unauthorized (snooping) name on my sons record by the name of *******************? Who is that?2.*********************************** ****** a receipts out of my hand and, and hand ripped the doc in front of me and my son. that was the second doc she hand ripped the other one was the release authorization form.3. My son documents were not completely filled out. There were some documents that didn't have his correct name, on it.4. his name was not blacked out on the documents.5.I was given a falsify invoice, and receipt. on the receipts the card holder it had another name other then mine(*************************).***** signed the receipts. She directed ********* to take the payment. When I asked why is this name on my receipt she said nobody never questioned that. I then explained The receipts was entered manually by *********. I also received 126 pages and was charged $79.00. The policy said $1.00 a page 6. *********************************** told me and my son to sit in the back of the office, because she has other families. which the office was not busy. Are we not a family of the doctors office?
