      **** reports the insurance company bought this.  She's asked several times for them to replace a worn out barring on a tire on her mobile scooter, she knows it's under warranty.  This is a safety issue.

                  We refer to the above and your
      recent correspondence about a complaint filed by **** *******. At Numotion, we
      take the privacy of our customers’ personal and health information very
      seriously. Numotion does not disclose information about any customer without
      authorization from that individual. Since a signed HIPAA release has been
      received, the details of Mr. ******* are as follows:
       
      The drive wheels were
      replaced on 8/7/23 (new solutions). Client called in on 9/28 and 12/13
      regarding wheel bearings making noise. During those times Numotion was no
      longer in network with client's insurance to which he was informed to seek out
      other company in network with ****** HMO. A new order opened on 1/30 where the
      client was willing to pay out of pocket. Eval was done in-shop on 2/9/24 where
      it was determined that lock nuts were loose and there was no need for
      replacement or warranty parts. Client signed service work order noting
      "both lock nuts on wheels were loose. Cleaned and tightened both and also
      applied Loctite", work order was labor only. Order was closed and billed
      as labor only on 2/9/24.

      Last December 2021, I received a power chair that was supposed to be custom made for me. It did not fit me, very uncomvortable, difficult to use, barely fits through my door and is overbuilt for it's intended use (indoors). When I told the technician I did not want it he said I could not continue to use the loaner I was using and I would have to go without a powerchair if I did not accept this one.

      Soon after that, I got Covid 19 and was very ill, almost died twice, and was out of commission for 4 months. Now I am regaining my strength and have had no results in my attempts to contact Numotion. I have called 16 times!

      I want a different powerchair that fits me and is made for my use. This is paid by medicare and I do not think I should have to pay for a new chair (this one had an invoice of $15,000).
