      Scheduled doctor appointment for 12/27/2024 @ 12:30pm Due to severe cough, chills, diarrhea, ear ache symptoms clinic schedule Virtual visit Upon time of virtual visit doctor was unable to get camera to work after 3 attempts. A plain phone call took place. Told doctor my symptoms of severe cough, unable to hear out my right ear, diarrhea since 12/17/2024. Doctor stated she can't determine exactly due to being over the phone. So prescribed antibiotics, inflammatory, anti diarrhea medicine. After receiving medicine it's did not work. I called office again 12/30 asking for work note and my symptoms had not cleared. Clinic refused to see me in person and had asked me the patient who's sick to pick up a doctor note for work, but refused to see in person to properly conduct an exam of my sickness. This has happened multiple times with the Craig location staff and doctors are highly unprofessional, only care about collecting their copays and not the health of their patients. The resolution is for an apology from the clinic for not fully their medical obligations towards any individual in need of medical care. COVID is not cause of every symptom in the world, doctors should be knowledgeable to determine the difference. Health care needs are serious and should not be turned down. I am still unable to hear or properly breathe and treating myself with over the counter meds due to negligence of the provider and it's clinic.

      The staff st Intermountain healthcare contributed to the declination of my ***** for 7months i had testing done in March of 2022 results given 10/25/23 after coming in multiple times for the dsme complaint and symptoms and now my diagnosis is so servere my organ csnnot be saved.
