    I recently changed my primary medical care provider since I moved from California to Nevada and acquired new employment.

    On 07/27/2022 I had an appointment with Elizabeth M*****, APRN at the Ralston Family Clinic here in Reno Nevada. I requested for my blood pressure medication to be re-filled and asked for ADA Reasonable Accommodation paperwork to be filled out on my behalf to provide to my current employer. I have a history of severe anxiety and depression and need to be out of work periodically while I have flare ups of my condition. I provided the Ralston Family Clinic with previous medical records from the Berkeley Family Practice Medical Group at **** ********* ******, Berkeley, CA as well as approved FMLA paperwork via email at Km*****@ralstonmedical.com. I have reached out 5 times (3 via email and 2 via phone) and have not heard back regarding status. I am required to submit this paperwork to my current employer so it is very important I get a response. I have been out on unpaid medical sick leave for approximately 2-weeks and need this paperwork to provide to my employer ASAP.
