    10/15/2022 Medical Records Denial Complaint To whom it may concern:On January 08, 2022, I (*************************** residing at *********************************************************** *****) visited ************** ************** who rendered a full eye exam, which included images of my eyes. On 10/15/2022, I visited ****************** office to request copies of my medical records. I was told that because my eye exam was normal, there were no records that could be provided. I specifically insisted and asked for a copy of the progress note which was denied by the office manager. She said all that she could provide was a copy of the receipt showing that I had images done. She refused to provide copies of the images and any other documentation associated with my visit. My visit was paid for by my insurance provider whom I know requires valid documentation such as a progress note to pay for the visit. I dont understand why I, as the patient would be denied a copy of my own records. Additionally, they tried to charge me $20 to sign a DMV form (mv619, a one-page document) certifying that they completed my exam on 1/8/2022. Although, HIPAA regulations impose a maximum of $0.75 per page to release medical records.I beg that you please assist in addressing this matter, so it does not continue to happen to other patients.*********, ***************************

    ****************** came into the office for a DMV screening for which our fee is $20.  He was informed of this fee and was unhappy about paying the fee and got irate and was unruly in the office, where my other patients didn't understand why he was being the way he was. One patient even said to me that he would have paid the fee for him just to calm him down. We gave him his prescription even though he doesn't have one. We would gladly send him a copy of his records to him. Its not something that we are able to do on the spot. The Doctor would have to write a report and mail to him and he would have to sign a release form. We have been in business over 40 years and he is trying to tarnish our reputation. He also said that we discussed his medical records in front of other patients which we did not.
