    G&i Ortho Supply, *** fraudulently charged my medical insurance carrier for intermittent urinary catheters with insertion supplies from April 2023 to December 2023. Total charges amounted to $19,900. I did not order these catheters. The insurance carrier paid the above amount and should be refunded.

    Claim submitted to ******** on Dates 7/12/23, 8/11/23 and 9/11/23 for 200 intermittent urinary catheters by G&I Ortho Supply *** ordered by ***************************. These items were never ordered and *************************** it not my provider. This is a fraudulent claim trying to get payment for items that are not for me under my ******** insurance. I need these charges removed immediately and want to file fraudulent charges. ******** has been made aware of this companies fraudulent practices. Attempted to contact this company the phone is disconnected and unable to reach to question these charges.
