      I had an appointment for a routine eye exam on 9/20/23. Upon checking in, I was advised that if I wanted a copy of my eyeglass prescription, I would be required to pay a $50 fee at check-out. There was also notice posted stating the same, there would be a $50 fee for a copy of a prescription. I questioned this as I never had to pay a fee for a prescription. I was told it would be submitted to my insurance carrier and if covered, I would be reimbursed. I asked again, if this fee was only to get a copy of my prescription and was told "yes". Upon check-out, I paid the $50 and was asked if I wanted a receipt. I said "yes". I was given a receipt (copy attached) which does not specify what the $50 charge is for. When I got home, I did some online research and discovered that the *** has what is known as an 'Eyeglass Rule'. Doctors are required to give a patient a copy of their prescription whether ot not they ask for it, free of charge. I would like my $50 refunded to me. Thank you.

      The patient consented to, and received a service that is typically not covered by medical insurance given coverage for eyeglass testing and resulting prescriptions are covered by vision insurance and no applicable vision insurance was presented at the time of the visit.  Our practice is to charge the patient for the measurement leading up to the resulting eyeglass prescription and not the prescription itself.   If we have a reasonable belief that the test is not covered by the patients medical insurance we charge for the test.  The patient is *********** receive the prescription free of charge, we charge for the non-covered test that results in the requested prescription.  I have attached a copy of the patients signed consent which discusses 'measuring for an eyeglass prescription' which is separate from a medical eye exam.

      With respect to the confusion caused by the front desk in relating the refraction to the prescription itself, we have refunded the customer.

      -when i made an appt, i advised i wanted an annual exam. ***** asked me if i had vision insurance or told me if it was in network -they also did not disclose they only do exams if there is a medical necessity -once exam was complete, no mention of a diagnosis -6 months later, i received a bill saying they diagnosed me with ***********, which there was no discussion of or treatment given -they also said i was near time going into collections even though i just received the bill. i spoke with several people in billing, including *********, ******** and *** -when i called asking if they mixed up my paperwork with someone else, an ******* ******** woman from the office called me being combative and said we dont do eye exams unless theres a medical need so whether they told you about it or not, we had to put it down for insurance
