    invoice ***** 8/1-8/29 for ******************* 8 years old billing for $74 for previously rendered services outside of allowed c overages from the insurer The provider has been in contract as an "in network" provider with NO co-payments for our daughter, **** for past 5 years The insurer, Emblem/GHi, allows treatments within defined timelines as well as allows if provider per-submits claims before treatment, this is nothing new and provider has been in business for several years and has knowledge of these restrictions placed upon them by the insurer provider decided to give added treatments outside of insurers regulations without consulting them first as well as not asking, me, the parent if we were willing to pay for said additional services "out of pocket"insure maintains the the provider, smile starters , has agreed to these given guidelines and provided treatment outside of the rules given them this being the fact, smile starters should accept responsibility for these charges as part of being an "in network" provider of Emblem/GHi smile starters needs to accept this responsibility and shoulder these said charges which were not emergency in nature and should have been aforementioned to an adult before providing out of network services

    We have contacted ************** numerous times in regards to his statement and have yet to speak to him personally for 2 weeks.  His insurance clearly states that he has reached the maximum benefits for those particular procedures rendered on 8/1/22.  As per our office's financial and practice policies and his insurance plan, the party is responsible for anything the insurance does not covered.  It is also the responsibility of the member to know their insurance plan coverage.  We practice under the guidelines of the **************** of Pediatric Dentistry and provide treatment following those guidelines above any patient's insurance plans.  ************** was inform of all the treatment that was recommended by the Doctor and consented.  We have checked with the insurance and the statement is accurate and owes the $74 according to his statement.

    Hi, We have spoken numerous times to ***************  We have tried calling him and it is very difficult for the Doctors to get a hold of him and speak.  ************** has found a new dental office for his child and that is wonderful because as long as the child is in good hands, we are happy.  We are not pursuing to collect the $74 from ************** since it has become very time consuming regardless on how much we explain the situation.
