    Beginning the week prior to October 17, 2022, I had to go see a cornea specialist. Upon seeing the cornea specialist I learned that I have two very back astigmatisms in BOTH of my eyes. Medically necessary contact lenses were the best option suggested to me, so I decided to take this route. I had received a confirmation letter from Davis Vision followed by an approval letter. Months had passed and I had repeatedly called them, as they had failed to pay for the medically necessary lenses. I had called multiple times and failed to receive one response, I had received multiple confusing responses. The lenses are $1,000 and they only covered $100. I had returned to my eye doctors office and they shared with me with all of the information that they had sent over to Davis Vision. There was no reason for Davis Vision to deny the claim and appeal that was sent over. Upon, trying to resolve the issue through Horizon NJ Health, I was told that they could not let me speak with Davis Vision. I was also told that the information my doctor sent over was incorrect, when it was correct, multiple times. Davis Vision refused to help me with this issue. My eye doctor also spoke with them and Davis Vision still neglected to pay for the medically necessary lenses, despite my eye doctor sending over the documents multiple times. Due to student loan payments, living expenses, and utilities I simply cannot afford to pay $900 at this time. This incompetence and negligence is extremely unacceptable, and I have been with them for years. There was no need to for this to happen, and it is has become as major inconvenience in my life, as I work full time and cannot see with my current old prescription as my eyes are so bad. I feel I should be compensated for this issue and I hope you can help me resolve it.

    Eye appt 1/13/23 went to optical department with prescription for glasses. Was told they see me on the insurance but my benefits were not populated. Davis vision has been trying to figure out the problem for 47 days as the problem is on their end. Ticket #:*******. Davis Vision Supervisor Patrice phone number ************.     This letter is in response to your recent inquiry, which was received by Davis Vision on
    03/03/2023, regarding a complaint filed with your agency by ****** ******. We appreciate you
    bringing this concern to our attention. Davis Vision strives to provide the highest level of
    customer service to our members and our providers, and we take all concerns seriously.
    As we understand ****** ******’s complaint, ****** states she has been waiting 47 days to
    submit and eyeglass order. ****** states the issue is that her insurance benefits are not active.
    Our investigation confirmed that ****** **** from ***** *** called Davis Vision Provider
    Services asking for assistance in placing an eyeglass order for ****** ******. ****** states
    she has been trying to submit the order since January 2023. The IT Department was contacted
    and problem reported under reference #*************. Challenge was resolved and ***** *** was provided an authorization number of *** ********* In summary,

    Davis owes mr $200.
    They said they sent the check.
    I called them three times.
    They never sent it.

    Good afternoon,  As we understand your complaint, you are dissatisfied that check number ******** had not been reissued to you.  Upon investigation, we performed a check tracer. The check tracer showed that this check was actually cashed on 06/21/22, which is why we have not reissued you a check. We apologize for any confusion, however, we are unable to reissue a new check for a balance that was already cashed. If you need proof of payment please reach out to Davis  Vision customer service.  Thank you!
