It has been over 2 months since I completed the requirements for a $1000 incentive offered by this company under its subsidiary "NationalRewardsClub" and mailed in documents to confirm my identity, including a W9. Despite this, not only have I not received payment, but the rep I was in communication w/has ceased replying to my emails. After mailing the documents in during April, in May I reached out to request an update. After several weeks and legal threats from me, the rep finally replied and requested that I email the documents. After I did this, the rep then responded stating that I should wait for 6-8 weeks for the form to be received via mail. This request was idiotic for 2 reasons. 1st, I had already emailed the forms as asked. Secondly, by the time he made that response, it was already 8 weeks after I had mailed the forms in. In addition, I also remailed the forms with tracking. Despite stating all this to him via email, at this point he ceased replying entirely. I write to warn all those who may come across this company & to find a resolution before I take this situation to court and the attention of the attorney general's office.Customer Answer

I completed 25 deals on consumer Digital survey. Which flex marketing does the payments for them when u complete the required offers. FLEX MG is saying that I've breached the rules, And I did not because they had already issued me a redemption for it because it was approved. That means they approved all of my credentials. So I'm entitled to my reward. I have a citibank card of a thousand dollars that was awarded Nov. 1st an I had til The Nov 24 2023 to mail in my APPROVED voucher they APPROVED. I have never won a award or used any creditials or address as they say. They must have me mixed up with another person as same name like b4 . I earned my award an I want my money.Business Response

Good afternoon,
My name is *************************, and I am the General Counsel at Flex Marketing Group, LLC. We have been in contact with *****************************. ***************************** contends that she is entitled to a $1000 prize; however, after running an analysis on our end, we found that she does not qualify for the prize.
***************************** received a redemption certificate pursuant to our website, Consumer Digital Survey, which we were required to receive by 11/24/2023. Upon receipt of her redemption request, we determined that ***************************** was a repeat winner.***************************** submitted requests on 8/15/ 2022, and 10/3/2022, and we sent payments for these redemption requests on 9/1/2022 and 11/25/20222,respectively.
Section VI of our Terms and Conditions clearly states that only one (1) account is allowed per household and that any accounts that share the same name or e-mail will be considered the same household. ***************************** has tried to circumvent this requirement by using different name and email address combinations; ********************************* on 8/15/2022, ***************************** ******************** on 10/3/2022 ************************ and now ***************************** *********************** The matching names alone were enough to disqualify *****************************; however,there is conclusive evidence that ***************************** submitted more than request.
To receive a prize, per Section IX of our terms and conditions, an account holder must submit a **9 with a redemption request. Upon inspection of the **9s for the redemption requests dated 8/15/2022 and 10/3/2022 we found that the Social Security Numbers were exact matches. Consequently, we believe ***************************** is trying to circumvent the rules once again by using a different address and email. Section IX of the terms and conditions states the following:
Any attempt to circumvent this restriction by attempting to register multiple times under different names, attempting to register more than one (1) time per household, and/or attempting to register more than one (1) time with the same mailing (postal) address and/or computer, is strictly prohibited. Failure to comply with any of these requirements will result in immediate termination of your participation in any Promotions and forfeiture of any Reward earned or accrued in connection with same. Please be advised that we have sole discretion to determine whether or not you have complied with the above stipulations.
Consequently, given the evidence on hand, in our discretion, we believe ***************************** is trying to register a third time and is not entitled to a prize. While ***************************** was successfully able to circumvent our process once, we are not entitled to redeem her request again.
If ***************************** shows a different social security number on her third W9, then we will make a determination as to whether ***************************** complied with the remaining rules and is entitled to a redemption prize.

I will continue to pursue legal action against the Flex MG and Consumer Digital Survey. My identity has been stolen 2 times as why I have IPIN for irs purposes . I don't run off my social security #. Per terms YOU SENT A "APPROVED " REDEMPTION CERTIFICATE STATED I WON THE ****** SO I AM PURSUING LEGA ACTION AS $1000 IS A MILLION TO ME. IF I DIDNT GET :APPROVED" THEN DONT SEND A REDEMPTION TICKET. Yes I completely understand. As this matter is in my Attorneys hands because of the identity theft. So he might be contacting you for the redemption money I am owed. IRS already issues me a IPIN number an if that's not telling you my identity was stolen not sure what other proof you need ? Furthermore, after identity case investigation I can provide more but he is jus goin to take care of my financial aspects , as I'm keep dealing with Companies/ individuals that don't follow on their part.thanks for your time

Ive completed the requirements for 1000$ gift card on **********************com Sent in my certificate along with my w9 form with my personal information on it and never heard from this business ever again.
