      I had to make an out-of-pocket deduction ($84.36) on Sep. 24, 2023, for the non-covered service of my blood test, and Quest Diagnostics has informed me that Weill Cornell has requested to proceed with the blood testing that my insurance does not cover. I also tried an appeal to UHC with Weill Cornell's provided letter that the tests were made for my essential testings, however denied.This was an unexpected expense and I should have known in advance from Weill Cornell and this wouldve not happened. I would like to kindly request a direct payment of the amount that I had to pay in order to do other urgent blood testing procedures. Weill Cornell ****************** has informed me to reach out to ************************ office. It's already been over half-year and after this incident and I have tried contacting Weill Cornell since then.

      I reviewed the patients case and confirmed with the patient the date of service as that was left out of the complaint.  Patient had required blood work drawn on 4/27/23 and the blood work was sent to a Quest diagnostic for processing.  All patients are advised that bloods will be sent to an outside lab for processing.  For the patient to have the treatment he was seeking at our facility the blood work was required before he can proceed..  As per the patient this was not covered by his insurance company.  Regardless the patient would still need to have the blood work done.  Unfortunately it's not a covered benefit with his insurance plan.  Patient will need to follow up with Quest and his insurance company.

      I would like to emphasize that the responsibility for this oversight lies squarely with Weill Cornell, as I provided all necessary information. I have already attached copies of the relevant documents, including billing statements, to support my claim. Weill Cornell failed to check if the requested blood tests were fully covered by the insurance. If not covered, Weill Cornell should have informed the patient (*********************) of the uninsured test. Unfortunately, I was told that all the blood test will be covered by my insurance in the beginning. Quest has informed that they already proceeded with what Weill Cornell requested, and UHC has informed that Weill Cornell did not check whether the blood tests were fully insured in advance.

      I understand that errors can occur in any administrative process, and I am willing to cooperate to resolve this matter amicably. However, the persistent neglect in addressing my concerns will leave me with no choice but to escalate this issue.

      Good Morning,

      Responding again to the complaint.  Patient had to undergo infectious disease screening as a part of routine workup and treatment protocols for infertility patients.  This blood work is medical necessary.  The providers office sent a letter of appeal letter on behalf of the patient, but the insurance still denied.  The patient would still be responsible for the balance with Quest diagnostic to move forward with infertility procedure.  Patient needs to speak with his insurance plan to find out why this isn't covered.

      I am repeating again, Well Cornell should have checked with UHC prior to submitting a request, the patient (myself) has no information what the clinic would be sending out to the bloodwork clinic. Weill Cornell has informed me that the bloodwork should be fully covered which Ive believed and unfortunately this falls under a misinformation towards a customer. I would like to again kindly request a bill adjustment to reimburse the uninsured bill amount. Also, I would like to avoid escalating this case to avoid any unnecessary burdening workforce.

      I am filing a complaint in regards to the bill for a $35 copay which was charged for my wellness visit with ********************************************************* on 10/26/2022. (*******************************, Guarantor #*********)When I made the appointment and met with the doctor I clearly stated that I was there for a wellness visit only.My insurance covers wellness visits 100% with no copay. During the appointment the doctor asked about any past health issues. I told him that I had had Lyme disease that had resolved itself aside from a slight tinnitus that DID NOT BOTHER ME.The doctor looked in my ears and told me that I had wax buildup. He informed me that I would need to see a specialist to have it removed because he did not have time to do so.At no time did ************************ inform me that our appointment was no longer considered a wellness visit. Under Section **** of the *************** Act, private health plans must provide coverage for a range of recommended preventive services and may not impose cost-sharing (such as copayments, deductibles, or co-insurance) on patients receiving these services. I am asking that the $35 copay be removed from my bill immediately. Thank you for your assistance and please don't hesitate to ask if I can provide anything further.*******************************
