    On September 14 I was approached by Stephanie S**** of 716ology and her husband at the ** ******** ** *** ***** They offered me a complimentary consult and a B12 shot to come as a new patient. I scheduled an appt for 9/16. At this appt I intended on going in for ***********. Stephanie suggested a peptide called ********** in addition. I paid $700 for the *********** and the upsell of **********. Stephanie told me the peptide would be in in about a week- her office would call me. I heard nothing. My follow-up appt for the *********** was 10/4. At this appt I asked about the peptide. She had no idea what I was talking about and looked through my chart for quite some time. She said that the peptide order "slipped through the cracks" but she would order and again it would be in in about a week. I still had not heard from Stephanie on 10/15 about the peptide so I sent a message respectfully asking for a refund since I still had not gotten the peptide and to cancel any future appointments. She had Ann, her office staff message me back stating there was a hurricane in FL where the medicine came from. She said she would refund the money minus the consultation fee (which was allegedly complimentary from the **** ********). On 11/12 I received a refund of $120. I called to discuss the amount I was refunded and she told me the $180 was the consultation fee. Mind you, SHE suggested the peptide at my complimentary appt. She told me I signed a contract stating no refunds on services and products rendered. I STILL DID NOT HAVE a rendered product!!! I wrote a negative review on ****** after being mistreated. In her rebuttal on the negative review she said the fee was for the 2 visits when I questioned a $180 ct fee. The refund invoice only states "Peptide Consultation Fee" which was an upsell and not requested by me! She also aid she refunded "her cost" of the medication, $120, not what it is sold for. It also says false info on invoice- that I received the medication. A call

    9/14– ** ****** *** ******** ***** open to the public. Heather was interested in our services and booked an appointment out of her own free will.  9/16 Patient came for initial consult. Due to HIPAA, I am unable to disclose any other aspect of services rendered. *********** was prescribed after her consult. *********** was received ($400 consult + additional $25 consult fee). Patient expressed interest in another service for Peptides ($300 consult + additional $25 consultation fee). Patient was aware of the costs associated. Patient was agreeable to the plan and paid at time of service. As per patient, she paid a total of $700. The additional consultation fees were waived, otherwise her charge would have been $750. 10/04 Second consult for ***********. Patient expressed that she was still waiting for her **********. Patient did not pay for any fees at this second appointment, despite services rendered. ($425 cost).   10/15 Patient had still not received **********. Unfortunately, there had been multiple Hurricanes in Florida and the southern US, causing severe shipment delays. Patient was updated with this however did not wish to wait any longer even though we had told her they should be received in the next day or two.  Patient paid $700 (original price $750) This included the peptide service and the *********** service.  Second appointment and consultation fee would have been $425 ($400 plus additional $25 fee), which was entirely waived by me. $700 total paid by patient.  Subtract $400 for the *********** Consultation, minus another $120 for my appointment charges from the second appointment. She was not charged a consultation fee for any appointment despite having discussed multiple services. This leaves her refund of $180.  Patient was upset because the ********** shipping was held up due to an act of severe weather. She had received all other products/services paid for.  Patient had called the clinic and unfortunately was uninterested in listening or participating in a discussion as threats were made by patient and hung up on our conversation. I apologize the patient is dissatisfied in our services and uninterested in communication. Thank you for bringing this to my attention
