    I am on a monthly medication (***********) that Tina Dubeck, FNP prescribed in March 2024. In Jan 2025, I started having problems with Tina’s lack of responsiveness. Tina Dubeck failed to coordinate the delivery of the medication and has not returned $925.00 I paid her for medication after several attempts to obtain the medication or receive a refund.
    On Jan 3, I contacted Tina Dubeck, FNP to inform her of my new address. I ordered 60 mg from Tina Dubeck, FNP in the amount of $650. Tina ran my card six times resulting in a hold of $3,900 on my account. I notified Tina.
    Feb 7, Received 30 mg (half of the order).
    Feb 8, Contacted Tina. She told me that I would automatically receive the second 30 mg in two weeks. I did not receive the 30 mg.
    Same day she indicated that while I wait, she could order the next 60 mg. Tina ran the charge on my FSA. She said it did not go through. I provided her our ******* card. She said it went through for $650.
    Mar 18, the 60 mg dose arrived from the February 8 payment of $650 on *******. I still had not received the 30 mg.
    Mar 19, I noticed a $600 charge on my FSA from Tina Dubeck on the same date as the ******* Card charge. I verified both ******* and my FSA paid Tina. I notified Tina.
    Between Mar 19 and Mar 30, I continued to text and call Tina Dubeck regarding the two orders. She did not respond.
    Mar 30, I sent a formal email re-explaining the situation, requesting either a refund or for her to make good on the order.
    Apr 11, Tina texted me and indicated that 60 mg would arrive on Apr 15.
    Apr 17, I notified Tina the prescription did not arrive.
    Apr 18, Tina responded that the pharmacy would begin producing *********** on Apr 22. She said orders would be shipped as soon as possible.
    Beginning Apr 30, I began contacting Tina every few days for a status.
    She did not respond to texts, calls, and emails.
    Tina Dubeck, FNP was paid $325 for 30 mg and $600 for 60 mg. I did not receive the medication. I am seeking a refund of the $925.00.
