    I had a colonoscopy with polyp removal on 12/18/2020. The polyp was sent to ******* Laboratories in ********* ** to determine the pathology. I have had full commercial insurance coverage with Aetna for the last 10 years and continue to have coverage today. I began to receive bills from ******* July of 2023. (Almost 3 years after the procedure).I called Aetna and *******. Apparently no claim was ever submitted by ******* to Aetna for the pathology reading after the colonoscopy. ******* is an in network provider with Aetna and per Aetna requirements - had 12 months to submit the claim. I have had conversations with ***** and ****** at *******-they have no explanation as to why the claim was never submitted to ***** The claim would have been paid in full by Aetna if they had submitted it in a timely fashion. ******* is now billing me the $400 directly. I would like this to be written off. ******* should have submitted the claim to ***** in a timely fashion.

    Upon reviewing the patients account, it was determined that the patients insurance was billed in 2020 for the service(s) in question. However,the claim was rejected by the clearinghouse/insurance and the rejection reason was Entity not eligible. This requires the use of an entity code.  This rejection reason typically signifies that the person who received the services was not eligible, according to the insurance company.

    Since this occurred during the height of the pandemic and we were processing thousands of COVID specimens, this claim was not followed up on until 2021. In 2021 the insurance company refused to reprocess the claim citing that the claim exceeded their timely filing limits. At this point, the claim should have been written off and the balance not pursued with the patient,which is an oversight on our part.

    We have now rectified the issue by adjusting off the entire claim balance effective 09/26/2023. ******* Laboratories, **** apologizes for any inconvenience caused by this issue and we have reached out to the patient to convey the same.
