      This used to be our pediatrician for our son until we moved out of state. We have had major issues getting ahold of them via email and phone to get a copy of our son’s immunizations records for his daycare and new pediatrician. Our son wasn’t able to attend day care because they wouldn’t respond and send us back completed paperwork about his health. Our new pediatrician now needs his immunization records and Bright Future is unresponsive after multiple attempts from me and his new new pediatrician. I’ve havent received a call back from them after leaving over 10 voicemails and have never received an email response after sending almost 10 emails. The one time I got ahold of them in August 2024 via phone they said to send them an email. Since then I have had no luck getting in touch with them. We now live out of state so I can’t go in person. I need them to respond to me or my new pediatrician and share updated immunizations records

      We received a transfer request on August 9th 2024 records were sent to physician for review and released to new medical office. Father emailed and called several times requesting we fill out a day care physical form for his child who at the time had already been transferred out of our clinic and out of state. We could not release any information to daycare without a release of records. We could not supply the physical report for the patient after transfer as his last physical was over a year prior to transfer. this was explained to the parents over the phone and not received well.
