      Dr. that I was supposed to be in my ******** network with the other doctors in this practice. Upon checking none of the staff said anything when I gave them my copay that they asked me for. So when I received the bill, it was an out of network bill. I called numerous times and the resolution is that I signed a disclosure that I am responsible for this bill. If this Dr **** a network Dr. then I should be notified who to see

      We are reaching out to the carrier to obtain confirmation as to our participation status with this plan. We are participating with *************** however we are being told this particular plan may not fall under that umbrella. We do believe we are par with the patients plan and will be having a conversation with *****. In the meantime we are adjusting the balance in full and the patient no longer has an outstanding balance. 

      Please see the attached zero balance statement.

      ENT and Allergy Associates fraudulently sent my information to a bill collector. I have paid every visit (they don't let you see the doctor unless you pay the co-pays up front). No one has ever called or contacted me for anything and today I received a letter from The Levinbook Law Firm stating that I owe ENT and Allergy Associates for a visit I had in November 2023. There were two separate notices - one for $20 and another for $30. They state that I have until June 21, ************************************** part of the debt. I have my credit statements proving that I have paid. Looking at other reviews, I can see that this is a habitual occurrence with this medical office.

      This patient owes $50 and is currently in collections for the visit on November 7th, 2023. There is no refund to be issued. 

       This patient was seen for two separate services on September 27th, 2023 and two separate doctors (specialties) on November 7th, 2023.

      Date of Service September 27th, 2023, total patient responsibility was $50 total ($30 for the Office Visit with the ENT, and $20 for the Audiology visit). Patient paid $30 at the time of the visit.  Patient owed $20 and paid this outstanding balance on November 7th, 2023.

      Date of Service November 7th, 2023, Patient saw the ENT. Total patient responsibility (see attached from insurance carrier) was $30. Patient still owes $30.

      Date of Service November 7th, 2023, Patient saw the Allergist. Total patient responsibility (see attached from Insurance carrier) was $50. Patient paid $30 on November 7th, 2023. Patient still owes $20.

       ENTA did send statements to the patient for the amounts owed  on [date] and [date] before the outstanding balance was sent to collections.
