  My mother had surgery and she was doing very well except for weakness. She was admitted to Chesterwood rehab around 8 PM 5/13/25. On the evening of 5/15/25, approximately 8 PM, my nephews took her a chair from home because Chesterwood never brought the recliner they promised. Earlier she told my sister she thought they gave her muscle relaxer, which was not one of her regular medication's and we requested that that be removed from her chart because it made her so groggy and weak. When my nephews arrived, they described her as totally out of it. They went to the car to get her forgotten blanket. One waited in the car while the other went in and witnessed her stand up and fall. My nephew looked all over the facility for someone to help him get her up when he finally found two people. He explained the situation and they told him that she wasn't their patient and they would contact her nurse. Eventually, they got her into the chair. I arrived at the facility on 5/16/25 around 12:25 AM. Upon entering the building and walking the very long halls to her room I did not see one person. I arrived to her room and the door was open right next to her room was a nurses station. There was a male nurse sitting there talking to a young female nurse aide. Upon entering her room, I noticed she was not in the bathroom nor the bed nor the chair in the corner. My mother was laying flat the floor with tube feed running. I called her name. I yelled her name. I shook her. I was crying sobbing because she was dead. She was cold and modeled. All of this commotion right next to the nurses station and no one came in. I yelled help I need help my mother's dead finally the male nurse slowly entered the room. 911 was called but it was decided it was too late. Police also arrived and lockdown the room until they finished an investigation and I do have that information. The facility left her in the floor, as if she was no more than a dog, my family placed her in the bed.

  Funny, nobody reached out to any of the family. And what do you suppose you can do to bring my mom back?? She wasn't even there 48 hours.
