    Insurance fraud and then running charges on credit card that were not approved by myself.

    **** has been a patient since 2007. Same protocols have been in place since 2003 & for her since 2007. Cleaning, x-ray and exam frequency's are the same. Please see information patient has signed acknowledging these protocols & frequency's. Also, see signed patient documentation reiterating patient is responsible for anything not paid by insurance. Last, see patient statement which states there is a 4% processing fee for credit cards, patient could have sent check payment. Not sure what the surprise is for the patient, but the claims are ridiculous. Dr. Kallmeyer's attorney will address fraud claim which is also ridiculous.

    They charged me for the X-rays 4 yrs ago per my insurance. The dentist office said they did not do X-rays in 6 yrs. I told the office what my insurance said and was told that was not correct. They billed my insurance for them and got paid. I feel they should write the balance of $96 off since it’s there mistake.
