      My father was in a care facility (********* ******) until the end of January 2024. He then moved into a care facility.

      We have received multiple bills from Internal Medicine & Geriatric Services and Crescent Health. Even though there are two businesses, they share a phone number for customer service.

      The bills in both cases are incredibly vague. The only "service" is "Daily Patient Services." In both cases, there are charges made after my father left the facility. He left on 2/1/24. There are charges for 2/5 and 2/7.

      We received a charge in March and paid it for $30. They sent a second charge for the same "services" which we received at the end of April. It showed that the charges were now increased and we owed another $30. No explanation of the change in charges. We also received a second bill for another $60. No explanation of charges.

      When I called to learn more about the services, I was told that I would need a PoA, which I have. ********* ****** has it as well. I submitted it on 5/6. As of 5/13, they have not "reviewed" the PoA and suggested that it could be weeks before it is done. I cannot match this to an insurance EoB because there is insufficient information to do so.

      I would like to get this information before paying any bills. At this juncture, this does not feel like a true medical institution.

      Please note that the patient was NOT discharged from the facility ( ********* ****** ) on 02/01 as the complaint suggests. He was discharged on 02/09/24 ( the facility has all the documents to substantiate this. He was seen by the MD  (Crescent Health, Inc) and her NP (internal medicine associates  who uses the same office and billing company but is licensed under her own incorporation).  Notes of service can be provided by request.

      I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this does not resolve my complaint.  For your reference, reasons for rejection are included below.

      My father was moved to ************* ****** on February 1st. There is NO way that he was still being treated at ********* ****** when he resided at a different facility. I can provide rent/tenancy documentation to show this along with witnesses. 

      My specific request was to receive the names of the MD and the NP who claim otherwise. And yes, I would like to see their notes. I requested this on the phone with them; however, they never responded and never provided any documentation to the family. 

      Given that Crescent Health has a D+ rating on BBB, I am going to need proof.

      The party submitting this complaint is not listed on the patient's record as someone that can be provided medical information to. Please note that if you are trying to obtain medical records they can only be provided to the two specific individuals that are listed on the medical record per HIPAA rules. Please request health or medical records through the individuals that are listed on the patients record. The patient was discharged from ********* ***** on 2/09/2024

      I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this does not resolve my complaint.  For your reference, reasons for rejection are included below.

      The company is stalling. The documentation was sent to ***************** on May 6th per the help line at this company. ********* ****** also has this on file. I would like to know what happened to this material that was sent over a month ago. There is no reason for them to not have this information. I have attached it again, but I'm sure they will have other reasons why they cannot share this information. 

      I have also included the Explanation of Benefits for the questionable transaction. It clearly says DO NOT BILL MEMBER. So why was the member billed? 

      If this is not resolved quickly, I will be including **** ***, *** *****, and the insurance companies on this matter.

      I received a bill from Crescent Health Inc., statement date 03/28/2023, for $56.40.
      I promptly paid the full amount.
      Yesterday, I received another bill from Crescent Health Inc., statement date 05/17/2023, for $30.37
      for the exact same services listed on the bill as were listed on the first bill.
      I called the number given for bill inquiries ###-###-####, and their explanation was that I had charges previous to the first bill that they applied part of my $56.40 payment to, and now I still owe them $30.37.
      I never received a bill for those supposed previous charges, and I believe they are defrauding me.
