    In January 2022, I enrolled in the *** (Flexible Spending Account) program offered by my employer, Lakeview Local Schools. I elected to have a total yearly contribution of $1450.00 ($120 is deducted from my check each month). I was never provided an *** debit card as requested, despite contacting our treasurer ********************* (and him contacting the company) numerous times. The company, according to **** in February 2022, said that they sent me a card. They never did. This led to me having to purchase things and mail in receipts for reimbursement. I mailed a reimbursement request for approx $300 back in May 2022, and it took them until July 2022 to send me a reinbursement check in the mail. I sent in another reimbursement request in August for several hundred dollars and have yet to receive a reimbursement as of December 16, 2022. When I reach out to this company, via phone, email, or their contact form on the website, they do not return my messages. At this point, I have taken on some credit card debt with interest, I (purchasing things that I believed that I would be reimbursed). I asked ********************* if I could stop my contributions, he said no since they are paid out for the year in advance. ***s only allow a rollover of $500 from year to year. Any money that is not used is forfeited. With that being said, I am about to lose hundreds of dollars of my hard earned money to this company. I do not have account balances available because they do not send me statements nor do they provide a way to access it all.

    You sent this complaint to Health Plan Administrators, LLC.  This business has not been active since 2019.  I have asked BBB to correct their records numerous times as I am not affiliated with WJL.  Again, please correct your records and contact the correct company.
