    7-26-2023 I filled out new paperwork with Time4U Massage Therapy. They asked me to fill out a paper with the name, address, phone number and debit card number, which I did. They stated the information would be inputted into the square system and then shredded. After the massage they asked me for payment after the card number was written down and I paid $45 with check #****. On 8/30/23 I received a phone call from a lady that owns Quilts Homemade in ********* stating she found a piece of paper with all my information written on it including a credit card number. I went to the store to pick this paper up when I noticed it was the same paper I filled out with Time4U. Approximately 1 and half weeks ago my card information was compromised and someone charged AnyTime Fitness Membership to my card. I had to pay to have it disputed and cancel that card. I went to Time4U and they just apologized and stated they shredded the paperwork. I took out the paper from my purse to show them my paperwork had not been shredded. The only response was it must have gotten stuck to another paper. They did not offer to refund me any of the fees, charge or anything.
