    June 24th We hired Bristol Hospice to make my Dad's( will refer to Dad as patient.) last days/hours comfortable at Forest ******* ******** care Facility. I provided and signed the paper work they required with a Bristol Hospice nurse provider for patient to be "admited" and qualified for hospice care due to Congestive Heart failure. I received a copy of the Hospice admission order. NO hospice nurse treated him. She evaluated him and advised it appears patient has hours left. The hospice nurse confirmed with the facility : the drugs Lorazepam and morphine the Hospice Dr ordered; the facility had them available on site. Patient never was treated with the signed order by ******** facility nurses said there was a pharmacy code was required to treat patient the ************* and the amount for the patient. The facility was only able to give patient the facility restricted amount of morphine which was .5ml every 15 minutes. The patient never received Loraepam. The patients family was by bedside and watched the patient suffer. The patient was in extreme pain and Hospice epically failed him and the family. The facility nurse on staff that day had approximately 15 plus other patients. It was impossible for the facility nurse to treat patient. This complaint is directed to Bristol Hospice. I request the bill to be cancelled. Thank you

    I have spoken to *****************************, facility DNS and pharmacy representatives to learn how this admission process and medication orders could have been completed better to meet the needs of this patient and family.
