    Amount from original billing statement $2321.94, has since been changed to $1406.94 (UNPAID)Account Number# ************** On 8/**/************************* sent a statement to my home attempting to collect for services rendered on 6/27/23. The statement sent to me was an attempt to defraud me of money and misrepresent my insurance coverage in a clear attempt to have me overpay for my procedure. This was verified after checking my explanation of benefits which confirmed Oregon Imaging Centers attempted to unbundle claims, which is medical billing fraud, and that these claims were then represented as valid in a statement sent to my home. I am aware that this was medical billing fraud as I have done insurance billing as part of my job for almost 13 years. Medical billing fraud is illegal. Unbundling claims is a violation of the False Claims Act as established by the ********** of ******** Not only did Oregon Imaging attempt to bill me for fraudulent claims, it misrepresented my insurance adjustments by fabricating a number on the statement, misleading me into thinking that funds were owed for those denied unbundled claims. The adjustments on the statement not only did not match my insurance statements but were also an entirely made up number. I contacted the billing department and was told an updated statement would be sent, without any acknowledgment of the billing fraud that occurred. I filed a grievance with Moda ************************* who have opened an investigation and taken my account of the events as well as all my evidence including scans of said statements. I have also contacted the ***** healthcare fraud reporting service, Oregon ********** of ******* Consumer ************** as well as the Oregon ***************** I would be happy to provide any supporting documentation as Oregon Imaging Center has deleted the original statement from their payment portal. My paper copies and scans are the only evidence that exists.
