    The provider repaired a broken retainer, which was paid for using an FSA health care credit card. I requested a detailed receipt showing the charges and the diagnosis codes as required by the *** for all qualifying health care related charges. The provider refused to provide a detailed receipt, even after multiple requests. I'm seeking a detailed receipt for the related charges as required by law.

    10/15/2024  Pt called stating his wife said they need a form to submit to insurance. I asked pt for his insurance info and let him know i can print a claim off and he can pick up. Pt said he will call back with his insurance info.

    10/15/2024  Pt called and provided Ins. info for ************ PPO ID#
    Pt was confused to what his next steps are, He said that ******* told her that she was going to submit the form on his behalf.. I let him know what our policy is and read the last note from *******. He then responded saying that he has now heard a couple differnt things now and that he needs to know exactly what his next steps are. I confirmed with Nat about pt having to come get the claim form and she will call to let him know hoe to proceed.

    11/05/2024 Called ****** today to inform him that his claim form has been sitting here for him to pick up since 10/15, pt responds that he didn't need it.

    I am rejecting this response because I repeatedly asked for a detailed receipt showing the diagnosis codes both when I initially paid for this service and again when I called and again asked for a detailed receipt, as required by the credit card (FSA Account) that I used to pay, and required by the ***. They told me they would not provide me with the detailed reciept; and they would only offer me the insurance form to submit to my insurance that they highlight here (completely ignoring my original and repeated request for the detailed receipt explaining the medical/dental services I received and the diagnosis codes). I have already paid the bill for these services so the insurance form does me no good. 

    By not providing the detailed receipt as required by law and as requested, they are not only violating the law, but are providing crappy customer service.
