      I have *** insurance through my employer. I made online appointment to Eye Health of Northwest PC; ***************************************. This is *** approved provider.I went on 1/26/24 at 8 am as scheduled. I showed my *** insurance card and online scheduled appointment .I was told to pay $ 95 upfront for routine first eye visit. I paid by my **** card .A optometrist saw me . Did exam and gave me prescription for glasses.Later on I saw on *** website that with my routine exam my copay should be zero. I contacted *** insurance ************ and spoke with claims person about this $ 95 charges . They tried to contact business ************ multiple times but did not hear back. I tried to call them ************ but have not heard anything back .The *** claim department confirmed that I do not have any copay for my annual visit and they also said they have not received any claim on my name from 1/26/2024 .I was wondering if this is some fraud going on in Eye Health Northwest ************** *****.

      Thank you for sharing this concern. The insurance information we received from the online appointment was Moda and not ***.  Therefore the $95 quote was based on the ************** benefits and not the ***.  We are happy to rebill the visit to the *** insurance and will contact the patient about rebilling and refunding the $95 payment.
