    I was in a skilled nursing facility for months after a back surgery for rehab. Insuring my stay, a podiatrist visited me and had to remove my large toe nail due to fungus. He gave the facility instructions to have a recheck in 2-3 months. My family and I were unaware the podiatrist was wanting to recheck my mail to image my toe and ensure the infection had not spread. Unfortunately, they ignored the doctors orders and I developed a severe infection and lost my large toe. We have proof they disregarded instructions and have lied multiple times about compensating me and even tried to hide evidence by withholding medical records.

    I have insurance with ******** and Champ-Va. I have been trying to get Prestige to *******************-Va properly. In order to *******************-Va they need to do this through the electronic system. (EFTPS) ************** does not send checks. I left this facility September 23, 2023. I talked with ******* @ their corporate office ************, she was going to *******************-Va correctly, however that didnt happen. I have called the Milwaukie facility left messages and have not gotten a return call. I do get letters stating they will be sending me to collections. The last one was signed by an RN *****************************? I talked with her and she stated she would have the billing person return my call. That did not happen. I called again, the billing person was not in, she only works there Monday and Tuesday. I was given her number ************. No return call. I called today no one answers the phone, which is typical for this facility. I have talked to Champ-Va numerous times, and they have tried to call the facility as well only to get no results. I dont want to be sent to collections over a $400.00 bill that they wont communicate on or bill correctly. *********************
