    To whom it may concern. In January 2022 I received a new Quantum Q6 Edge iLevel Wheelchair (Fully Loaded) - Seat Lift, Tilt, Recline & motorized Legs, purchased by the ****** VA Hospital. I’m a veteran that is a double leg amputee and in hispid care. I had a choice of choosing a Quantum Q6 Edge iLevel wheelchair or a ******** wheelchair. I chose the Quantum Q6 Edge iLevel Wheelchair.In Jan. 2022 I was already in a ******** ** wheelchair that had become broken. The ******** ** was just a wheelchair with tilt. There was no lift/recline/motorized legs.Upon me receiving my new Quantum Q6 Edge iLevel Wheelchair, a whole new world opened for me and my family. I was now able to cook for myself, I could reach things in the refrigerator, I could get my own glass on water for the first time in years. I was now able to do transfers without a **** lift. I could reach things in cabinets, my mobility increased 1000%.Sadly, at the end of August 2020 my Quantum Q6 Edge iLevel Wheelchair broke. I was stuck in the wheelchair and my family had to use a **** lift to get me out the wheelchair.Im now basically stuck in bed. No mobility, I now have to have family wait on me for everything. I don't have the strength and agility to operate my manual wheelchair. Upon me contacting the ****** VA hospital, I was advised that they would assign the chair repair to ****** ******* * ******** of Denver, *** * *** ***, Denver, Colorado *****.Phone ( ###-###-#### ). ****** ******* * ******** of Denver sent out a tech/repairman named ***** who evaluated and inspected my chair, he related the motors had burnt out and he would order replacement parts. The service rep at ****** ******* * ********, *** *** ******** {***************************} related the parts are back ordered and they didn’t know when or how long before they could get the parts to repair my chair. This process has become unacceptable. The wheelchair is under full warranty and needs to be repaired or replaced immediately. Thanks

    From the Home screen on any display on the Q-Logic 3 joystick, go into Auxiliary
    Select System Information
    Choose Provider Information
    Look for Unit Serial Number

    Please reopen this complaint. There hasn’t been any resolution to the complaint.
    The requested information is that the wheelchair was ordered by the ****** VA hospital in November 2021. The wheelchair was delivered in January 2022. The serial # is **************.
