      In net work Dental provider collect money prior to submitting claims to dental insurance, told me I would have to get reimbursement from insurance company. Then never filed for entire claim because I already paid. Provider knowing they would have to accept agreed amount for procedures and instead over charged me than the agreed amount with in net work providers. Then submitted claims to insurance company after I inquired about ****. Dental provider double billed and was not to charge me for procedures because of being in net work.

      To Whom It May Concern:

      We have received the complaint filed by Mr. ********** However, this letter states that you 
      attempted to contact us via email or mail previously. We never received the original 
      complaint, and I do apologize for the late response.

      Our office has been working with ************************* to receive payment on date of 
      service: 9/6/2024. We are aware that Mr. ********* has most likely overpaid for the work 
      that was rendered on this date of service, and we are diligently working on finding out how 
      much he overpaid.

      I spoke to a representative of Mr. ********** dental insurance on 1/22/2025 and have been 
      working with them to get this issue resolved. We are still awaiting payment for these 
      services, and upon receiving payment from his insurance company Mr. ********* will 
      receive a refund for his overpayment. I have been advised by his insurance company to 
      wait until we receive final payment before issuing a refund to Mr. ********* as this will 
      determine the amount that is due back to him.

      I am covered by ***** dental insurance. The claim was paid by *****. I owed nothing according to *****. This office insisted I owed one hundred sixty eight dollars. I paid the bill to keep the office manager from complaining to me. ***** was contacted about this problem and was told that I owed nothing according to the provider’s contract with *****. I have requested a refund but have heard nothing from MDA. I have the receipt from my payment. How do I get my refund?

      I am filing a claim because:1. I had charges added to my dental bill more than a month after a service was provided for no additional services or appointments.2. The office sent me to collections for a bill I have made inquires about with no response from them.I had dental work on several different occasions this year. The last dental work billed for took place on 2/15/2024. I have a dental insurance plan that is 100% coverage up to $1500 each calendar year. When leaving that last dental appointment I stopped at the front desk and asked to pay the difference which would have totaled $142.28. The dental office said I did not owe anything that day. Several months later on May 23rd, 2024 I received a call from my dental office telling me I had a balance. The balance was $431.63. The office said my dental coverage ran out and I owe the difference, but $431.36 was not the difference. I asked for an itemized ledger and received one electronically. I have never received an actual bill from the office in the mail or electronically to date. On the ledger there is an adjustment charge added on 3/26/2024. The description is "Patient Adjustment- Increase Patient Balance" for $289.35. They said because my dental ran out they can charge the difference and add on additional charges even without additional dental work. I have called Modern Dental Associates 4 times from 5/23/2024 through 6/24/2024 to inquire about the additional charges for no additional services. Each time they cannot answer the question and tell me they will call me back but do not. They already billed insurance for the amount for services and dental work. Not until after insurance coverage ran out did they tack on more charges. Today, when contacting them they said they are no longer able to answer questions about the bill because they sent it to collections. They never responded to my inquiries on 4 occasions and still will not answer questions about the charge.

      Modern Dental Associates is now refusing to answer my calls, and they have not returned any previous phone calls for the past 4 weeks each time I tried to call and inquire about the balance to pay it. They emailed me today finally and said they showed me the charge once in May and explained the additions even though they did not, and that they already sent it to collections. I have never received a bill from Modern Dental Associates and each time I called about the balance they said they would call me back and failed to do so. They still have not fully answered my questions about the balance and now will not tell me where the sent the balance so that I can take care of it like I wanted to over the phone the last 4 times I've called.
