    ADCGC agree to provide consultancy service for ******** adult daycare center. The consultancy service ?was not provided as per agreement and there was so much delay in the process due to not providing ?correct document and consulting service to the Chalfont adult daycare center. Below is the list of the ?issue:?1. ADCGC consultant did not come to two township meeting in the year 2022.?2. Daycare initial application was not completed appropriately and that delayed my application process.?3. Architecture drawing was no reviewed by ADCGC and I get approval for less number of client due to ?consultant did not provide guidance to change the bathroom as per state regulation to ensure we get ?approval maximum number of clients from state.?4. No consultancy provided between interim license and final inspection and ADCGC denied coming to ?site for pre inspection prior to final state inspection.?5. Consultant left in the middle of the inspection during both state inspection.?6. Chalfont adult daycare received two citations from state final inspection and ADCGC denied not ?provide support for citation plan of correction.?7. State inspector provide some comments that all policy and procedure are required to update language ?that is not compliance with state regulation. ADCGC denied correcting the policy and procedure.?8.Promise ******** application required document was not provided for 7-8 weeks and OLTL form was ?not filled out correctly. 9. Training manual provided does not have all required training material content as per state regulation requirements. ADCGC denied providing all required training material. 11. ADCGC consultant use inappropriate language in email whenever i point out error and requested the required documents for the project. 12. Architect approved drawing was uploaded to ADCGC website without my knowledge. 13. ADCGC consultant do not have appropriate knowledge to provide the service and she is fraud. I want ADCGC consulatant to refund my money.

    The town township meetings were regarding the change of use variance with zoning. My contract states ADCG will assist in zoning. As a consultant, I recommend a zoning attorney since this group of 5 partners from ******** adult day Care were very young . I found them a local experienced zoning attorney. They said they wanted to do it themselves and if they got into trouble they would get a zoning attorney. ADCG specializes in licensing adult day care not zoning. They failed on first attempt. Again, I recommended a zoning attorney. They denied. Another 2 months went by waiting for a new zoning hearing and now the landlord wanted them to sign a 10 year lease with no zoning. ADCG recommended not to sign the lease until they got zoning and get a zoning attorney. The Partners at ******** adult day care signed the lease anyway and went to another zoning meeting. Again denied. They called the zoning attorney I suggested, had him explain what was happening, did not pay the zoning attorney nor use him to represent them, went to zoning a 3rd time and got their zoning. ADCG Assisted them in zoning as per the contract.

    2.  The adult day care application . 

    Answer: The adult day care application for the department of aging in Pennsylvania is a group effort including all partners, five in the this case, to get check ups and background checks. This group went to an urgent care to get check ups which was not accepted by the state licensure. They had to go to their own doctors. ADCG had no knowledge this ******** adult day care partners would not go to a regular doctor. ADCG worked diligently with all partners in order to get all partners background checks, check ups, employee resumes, TB tests . ADCG cannot force all partners to get this information together on timeline which was never stated to ADCG. ADCG’s part of the application was provided four months ahead of the application submission. ADCG had to resubmit material to customer and four other partners because they did not read the material or keep emails.

    3.  Architectural drawings not approved. 

    Answer: ADCG was not invited to work with the architect or landlord on this project. Therefore, ADCG cannot take credit for this mistake. ADCG informed Customer and partners that they should have another bathroom but the suggestion was not taken.

    4.  No consulting during pre-inspection. 

     Answer:ADCG drove the hour it took to arrive at ******** Adult Day Care from their offices at all requests from Customer and partners for one and a half years. ADCG was not requested to appear after first inspection which ADCG was there for. Before first state inspection, Customer grew increasingly aggressive in mental abuse and displeasure of ADCG material which got him licensed. Customer wanted everything done for him by ADCG so he did not have to do anything. ADCG does not sell franchises where everything is done for you. ADCG is a health care consulting business that provides all paperwork and assistance to get an adult day care license which is how ******** adult day care got licensed. ADCG did not meet Customer for 5 months since the contract was signed. Another partner hired ADCG and ADCG still has a good repour with that partner. Further, ADCG was not contacted by Customer or any other partner to be present after first interim license. ADCG are not mind readers. ADCG was contacted to be present at last survey for permanent license and did appear.

    5.  Consultant left in the middle of both state inspections.  Answer: Consultant stayed until the Surveyor said she was finished at first state inspection. Consultant , who attended, has a broken knee and stayed seated, in pain, for as long as possible. She put off knee surgery to be present for ********’s two inspections.  Upon second inspection, consultant stayed for 2 hours and saw that she was not needed and due to pain left.

    6.  ******** received two citations. 

    Answer ADCG is always there ready to support and correct any citation the customer will receive from state whether they are ADCG’s fault or customer’s fault. I was not notified of the needed support. 

    7.  Policy & Procedures language. 

    Answer: Upon a visit to ******** Adult Day Care, Customer showed ADCG one page that needed language correction and it was done by ADCG at that moment. Customer never gives all pertinent information to ADCG and consequently makes ADCG’s job impossible to help him. According to only Customer from this group of 5 partners, ADCG is always wrong.

    8.  PROMISE APPLICATION. 

    Answer:  ADCG offered to completely fill out form for Customer who said he couldn’t do it. He refused help and did it himself or with partners. OLTL (office of Long Term Living) was contacted by email by ADCG 4 times to get a form and never heard from them. ADCG called twice. Once the OLTL employee said there was no such form. The second time ADCG called they received form right away by email. ADCG sent form as soon as they received it to Customer group email. Customer was given OLTL email and phone number but made no attempt to contact them. 

    9.  Training Material. 

    Answer:Contract states Training material/curriculum will be supplied and it was or else ******** wouldn’t have gotten their license. 

    10.                Language 

     Answer:Any language used was appropriate for business.

    11.                Architects drawing. 

    Answer: ADCG places all centers they license on their PROJECTs page. Customer said he didn’t want that so it was taken down immediately.

    12.                Fraud. 

    Answer:ADCG has been in business 25 years and has never had a lawsuit. We start adult day care centers nationally and the adult day care consultant who helped on this project is also an expert witness for adult day care wrongful death cases. If ADCG is a fraud ******** would not have gotten a permanent license. Further, out of 5 partners, only Customer is upset with ADCG material.

     

    13.                 Customer now has a permanent adult medical day care license from Pennsylvania for which ADCG was hired to obtain. Customer and partners have all paperwork as hard copies which they have read and assimilated in their possession. Therefore, ADCG does not believe customer needs their money back.

    I am rejecting this response because:


    State representative stated all policy provided is not compliance with state regulation during inspection. Training material also missing all required training as per state regulation. The documents provided not well organize not even has pagination correct. The reason we hire consultant to ensure complete the process on time but we got delayed 5 to 6 month due to consultant did not provided service on time.

    We are still waiting from consultant response for Promise application (OLTL) received correction. As per attached correction checklist all the
    policy provided by consultant does not meet the requirements of 55 PA code. I
    called consultant but no response. Also, I emailed the observation checklist to
    consultant, but no response received as of today. I have to hire new consultant.

    Hi,

    1. Lease was signed after ADCG consult stated building is goof
    the adult daycare business, but which was not true. We got denial from township
    and have to file variance. ADCG consultant promise to come to township meeting
    and did not come to hearing meeting and text us that she cannot be able to make it for both
    hearing meeting. 

    2.  The adult day care application . - Consultant did not review all documents appropriately and we received observation. Consultant did not provided service as per requires state regulation and thats what caseu the issue for delaying the process. Consulatant job was to reveiw application and provide guidance before we submiot the application. 

    3.  Architectural drawings not approved. - The response is lie - all Architect drawing was submitted to ADCG for review. she even upload my approved architect drawing to her website without my consent. No suggestion was provided to change in approved drawing. 

    4.No consulting during pre-inspection. - ADCG was contacted and notified for final inspection date. She denied to come for review all documents and building for final inspection. Consultant did not even review anything to get us ready for final inspection. 

    5. Chalfont received two citations. - there is email attached in my original complaint her denial for not to provide service for citation received. 

    6. PROMISE APPLICATION. : Consultant should have knowledge for application process. i did not pay money for to wait from OLTL office. It took 7-8 weeks to just complete application. 

    7. Training Material. - there is email from state inspector that training material provided did not have all required content as per state regulation. 

    8. Promise OLTL application received observation checklist from state. It state none of the policies submitted does not meet the requirements of 55 PA code. This observation checklist was sent to ADCG consultant and no response received as of today 12/07/2018. the due date for response is 12/18/2023. - after waiting 7-8 weeks all policy provided are not compliance with state regulation. see attachment. The problem is still not ended. 

    9. Once I receive OLTL approval consultant has to proved service to apply for contract for MCO. which i asked her to start working on this process to get ready all require document but no response received. 

    The reason to hire consultant to ensure all process completed on time with minimum observation. We have wasted so much time and money due to ADCG fraud and not providing right service on time. i am requesting to refund all my money.
